3Het brandwondenportaal is een e-Health applicatie voor patiënten met brandwonden. Patiënten vinden het brandwondenportaal waardevol, met name omdat duidelijke en betrouwbare informatie op elk moment beschikbaar is. De littekenkeuzehulp helpt hen en de zorgverleners in het samen maken van een behandelkeuze. De mate van samen beslissen is na de introductie van het portaal gestegen. Met name bespreking van de manier waarop en in welke mate een patiënt betrokken wil worden bij het maken van een beslissing is verbeterd. Net als het samen bespreken van de voor- en nadelen en het samen afwegen van verschillende behandelingsmogelijkheden. Wensen voor doorontwikkeling van het portaal zijn onder andere informatie speciaal voor kinderen en hun ouders en ervaringsverhalen.

Brandwonden zijn een van de grootste fysieke trauma’s die een persoon kan oplopen en kunnen gepaard gaan met grote wondoppervlakten en veel pijn. Daarnaast kan een persoon met brandwonden ook een psychisch trauma
oplopen. Het bewust meemaken van het ongeval waarbij kleding en haren vlamvatten of het gevoel hebben het ongeval niet te zullen overleven kan een grote impact hebben op iemands leven. Het kan leiden tot een gevoel van hulpeloosheid en onveiligheid en daarmee tot acute stresssymptomen die zich verder kunnen ontwikkelen tot een posttraumatische stressstoornis (1). De internationale cijfers over hoeveel mensen met brandwonden ernstig last
hebben van acute stresssymptomen en posttraumatische stresssymptomen variëren tussen 3% en 45% (2). Nederlands-Belgische cijfers laten zien dat ongeveer een op vier patiënten tijdens de opname in het brandwondencentrum
hiervan last heeft (3).

Het oplopen van brandwonden is voor kinderen én hun ouders een ingrijpende ervaring die kan leiden tot posttraumatische stresssymptomen. Ouders voelen zich vaak schuldig en machteloos wanneer zij hun kind zien lijden door de pijn van de brandwond. Ook de wondverzorging in het brandwondencentrum kan traumatisch zijn: het kind ervaart angst en pijn, terwijl de ouders geconfronteerd worden met het leed van hun kind. Vanuit de pediatrische medische traumaliteratuur wordt aangenomen dat deze stressvolle omstandigheden ook bij kinderen kunnen leiden tot posttraumatische stresssymptomen.

BACKGROUND: Following a severe burn injury, individuals embark on a lifelong process of managing and integrating the physical, psychological, and social consequences, including functional limitations, fatigue, altered body image, and psychological trauma. To aid in this process, healthcare professionals can provide self-management support. To date, however, no self-management support intervention has been developed to meet the unique needs and preferences of burn survivors within the context of burn aftercare. In this article, we describe the process of developing a self-management support intervention for burn aftercare and present the resulting intervention.
METHODS: A structured, multi-stage process was followed from May 2021 to December 2023 to develop the intervention, guided by established frameworks for the development and adaptation of complex interventions. The process included evidence review, stakeholder consultation, and participatory observations to identify needs, inform design decisions, and ensure contextual fit. A hybrid approach to intervention development was adopted, combining adaptation of an existing intervention with augmentation through five co-creative workshops involving burn survivors, healthcare professionals, researchers, and burn care decision-makers. The final prototype was refined through expert reviews and real-world pilot-testing to assess its feasibility and acceptability.
RESULTS: The structured, multistage process resulted in a self-management support intervention addressing the physical, psychological, and social needs of burn survivors. The intervention was named BreeZe (Brandwonden en Zelfmanagement/Burns and self-management). Intervention Core components of the intervention include a holistic care approach, goal setting and action planning, solution-focused brief therapy, motivational interviewing, case management, and the acknowledgment and involvement of informal caregivers. BreeZe is supported by materials like the Self-Management Web, a patient booklet, training for healthcare professonials, and a comprehensive manual. BreeZe is delivered by trained healthcare professionals and is structured into five phases, focusing on holistic needs assessment, goal setting, progress monitoring, and ongoing motivational support.
CONCLUSIONS: The structured, multi-stage development process was thorough and carefully considered, balancing diverse stakeholder perspectives with scientific evidence and theory. BreeZe offers an evidence-based model tailored to the Dutch context that can be integrated into routine practice that shows the potential to enhance the effectiveness of care and support a more collaborative, patient-centred approach.

Voor het instellen van de optimale therapie van brandwonden – conservatief of operatief – is een vroege, accurate bepaling van de brandwonddiepte belangrijk. ‘Laser Doppler imaging’ (LDI) is een techniek waarmee een nauwkeurige inschatting van de brandwonddiepte kan worden gemaakt door het meten van de dermale perfusie. Hoewel is aangetoond dat de keuze voor het wel of niet verrichten van een operatie met LDI eerder kan worden gemaakt, heeft dit niet geleid tot een kortere tijd tot wondgenezing of kostenbesparing in de Nederlandse brandwondenzorg. LDI wordt in alle Nederlandse brandwondencentra gebruikt. Bij twijfel over de brandwonddiepte in de eerste of tweede lijn is doorverwijzing naar een brandwondencentrum raadzaam.

Tijdens een fietsvakantie in het Engelse Cornwall, samen met zijn vrouw Ellen Witteveen, liep Ger Monden uit Amsterdam ruim tien jaar geleden hersenletsel op door een val. Na een half jaar in coma onderging hij intensieve revalidatie. Ellen merkte dat zij vaak onvoldoende bij dat proces werd betrokken. ‘Als mantelzorger ben je veelal geen interessante partner voor de medische wereld. Sommige professionals ervaren je voor mijn gevoel toch meer als concurrent als je een aandeel in de behandeling wilt hebben. Ik ben docent en onderzoeker bij het Kenniscentrum Sociale Innovatie van de Hogeschool Utrecht. We doen onderzoek naar mensen die langdurige zorg nodig hebben.

De meest gebruikte techniek om brandwondendiepte te bepalen is de klinische inschatting. Dit is een goedkope techniek, maar niet de meest accurate. ‘Laser-doppler-imaging’ is een techniek waarmee, door van het meten van de dermale perfusie, een accuratere (> 95%) inschatting van brandwondendiepte kan worden gemaakt. De hypothese is dat de introductie van laser-doppler-imaging zal leiden tot een snellere keuze voor het wel of niet verrichten van een operatie, met mogelijk een kortere ligduur en lagere kosten tot het gevolg. Om deze hypothese te testen, loopt er momenteel een gerandomiseerd gecontroleerd onderzoek in de Nederlandse brandwondencentra.

Background After a burn injury, the survivors have to manage and integrate the physical, psychological, and social consequences of their injury into their daily lives, such as functional limitations, aesthetic complaints, and fatigue. How successful survivors of burn injuries are at this depends on their self-management skills. Health care professionals play an important role in supporting the self-management of survivors of burn injuries. Currently, there are no burn-specific self-management support interventions. Therefore, we developed a self-management support intervention for survivors of burn injuries, called BreeZe (Brandwonden en Zelfmanagement). Objective This study aimed to describe a study protocol to implement and evaluate the BreeZe intervention. Methods This multicenter study in the Netherlands is an implementation-effectiveness hybrid type 2 study, with a nonrandomized stepped-wedge design. Starting April 2024, 3 phases have been sequentially rolled out across the 3 specialized Dutch burn centers over a period of 20 weeks—the preimplementation phase (usual care), implementation phase, and postimplementation phase. To identify barriers and facilitators of implementation, the Consolidated Framework for Implementation Research (CFIR) will be used. For evaluation, the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) evaluation framework is used. The coprimary outcomes are (1) self-management skills and (2) the implementation outcomes are reach, adoption, implementation, and maintenance. Secondary effectiveness outcomes are self-regulation, participation, dependency, patient-centeredness for survivors of burn injuries, self-management support skills for health care professionals, and cost-effectiveness. Data collection for survivors of burn injuries occurs at 2 weeks, 6 months, and 12 months post discharge, using questionnaires. Data collection for health care professionals occurs before training and 3, 6, and 12 months post implementation, using questionnaires, video observations, and interviews. Data analysis will include both quantitative and qualitative methods for comprehensive evaluation. Results Participant recruitment ended on June 30, 2025. Follow-up data collection ends in July 2026. Conclusions This study will evaluate both the effectiveness and implementation of the BreeZe self-management support intervention for survivors of burn injuries using a hybrid effectiveness–implementation design. By applying the CFIR and RE-AIM frameworks within a stepped-wedge design embedded in routine burn aftercare, this study aims to generate robust and practice-relevant evidence on how self-management support can be effectively implemented in burn care. The findings are expected to inform both clinical practice and future implementation efforts in burn aftercare settings. OBJECTIVE: The objective of this study was to identify the prevalence and development of after burn joint limitation by scar contracture.
METHODS: In 2011-2012, consecutive patients were enrolled in this prospective multi center cohort study. Eligible were all patients admitted to the 2 participating Dutch Burn Centers with acute burns across or adjacent to the neck, shoulder, elbow, wrist, hip, knee and ankle. Passive range of motion was measured in week 3 and subsequently every 3 weeks until discharge, on discharge from the hospital and during follow-up at the outpatient clinic at 3-6-9-12 months after burn.
RESULTS: Limited range of motion of non-operated burned joints (N = 195) was restored back to normal within 6-9 months. From the operated burned joints (N = 353), 58.6% demonstrated a limited range of motion at 3-6 weeks declining to 20.9% at 12 months. The upper part of the body was affected more often by scar contracture than the lower part. At 12 months, the shoulder was limited most often (51.3%) and the hip least often (0%). Reconstructive surgery was performed in 13.3% of the operated burned joints.
CONCLUSIONS: Persistent joint limitations at 12 months were exclusively present in joints that needed skin grafting for rapid wound closure. The upper part of the body was more prone to contracture formation than the lower part, from which the shoulder was most often involved. More than half of the limited range of motion seen in the acute phase, resolved in the long term. The need for reconstructive surgery was less than expected.
