Er bestaat een nauwe samenhang tussen de smaak- en de reukperceptie.
De smaakperceptie vindt plaats door smaakpapillen die zout, zuur, zoet, bitter en umami kunnen onderscheiden. Daarnaast kunnen 2.000-4.000 verschillende geuren worden herkend. Veel smaakstoornissen zijn in feite reukstoornissen. Speeksel beïnvloedt de smaakperceptie omdat het fungeert als oplosmiddel voor de smaakstoffen en omdat het de smaakreceptoren beschermt. Daarom leidt een vermindering van de speekselproductie gewoonlijk tot afname van de smaakperceptie, waarbij de concentratie van zinkionen en specifieke eiwitten in speeksel een belangrijke rol spelen. Ook zink- en ijzerdeficiëntie kunnen leiden tot verminderde smaak- en reukperceptie.

Pijn aan het bewegingsapparaat staat in onze samenleving op de eerste plaats van oorzaken voor ziekteverzuim. Bij aspecifieke chronische lage rugpijn kunnen lichamelijke inspanningen op het werk zoals dynamische en statische functionele capaciteit verlaagd zijn en de aanleiding zijn voor ziekteverzuim. Volgens het promotieonderzoek van Sandra Jorna-Lakke is bij deze groep patiënten aanvullend onderzoek nodig door middel van gestandaardiseerde functionele capaciteitstesten.

In het vorige deel van dit tweeluik zijn we ingegaan op hoe je met een conservatieve bulk spiermassa kan opbouwen, terwijl je de aanwas van vetmassa zoveel mogelijk beperkt. Na een zogenaamde cleane bulk wil je graag de zorgvuldig opgebouwde spiermassa ontdoen van wat vetmassa. Het verliezen van vetmassa, terwijl je je opgebouwde spiermassa zoveel mogelijk vasthoudt, wordt cutten genoemd. Hoeveel spiermassa je verliest tijdens een cutfase is onder andere het gevolg van hoe diep je onder je energiebehoefte duikt. Verliezen van te veel spiermassa is echter niet het enige grote nadeel van een slechte cutfase. Ook je fysiologie en humeur kunnen een stevige knauw krijgen als je verkeerd cut. Wij proberen op een rijtje te zetten hoeveel energie, eiwitten en koolhydraten je eigenlijk nu nodig hebt en waar je nog meer op kan letten in een fatsoenlijke cutfase.

BACKGROUND: Anthropometric measurements are widely used to reliably quantify body composition and to estimate risks of overweight in healthy subjects and in patients. However, information about the reliability of anthropometric measurements in subjects with severe intellectual and sensory disabilities is lacking.
OBJECTIVE: The purpose of this study was to determine the feasibility and the test-retest reliability of body composition measures in subjects with severe intellectual and sensory disabilities.
METHOD: The study population consisted of 45 subjects with severe intellectual and sensory disabilities. Body mass index, waist circumference, skin folds and tibia length were measured. Reliability was assessed by Wilcoxon signed rank test, limits of agreement (LOA) and intraclass correlation coefficients. The outcomes were compared with values provided by the World Health Organization.
RESULTS: There were no significant differences between test and retest (P < 0.05). For the skinfold measurements, however, the LOA was insufficient. Intraclass correlation coefficients for all variables, except skinfold measurements, were 0.90 or above.
CONCLUSION: Test-retest reliability and feasibility for all measurements are acceptable in subjects with severe intellectual and sensory disabilities. Skinfold measurements, however, could not be reliably performed in these subjects. Measuring tibia length and using the determined formula to calculate body height from tibia length is a reliable alternative for measuring body height. Although measuring the body height of subjects with severe disabilities was feasible, measuring tibia length was more feasible.

Caregivers of persons with profound intellectual and multiple disabilities (PIMD) often describe the quality of the daily movements of these persons in terms of flexibility or stiffness. Objective outcome measures for flexibility and stiffness are muscle tone or level of spasticity. Two instruments used to grade muscle tone and spasticity are the Modified Ashworth Scale (MAS) and the Modified Tardieu Scale (MTS). To date, however, no research has been performed to determine the psychometric properties of the MAS and MTS in persons with PIMD. Therefore, the purpose of this study was to determine the feasibility, test-retest reliability, and interrater reliability of the MAS and MTS in persons with PIMD. We assessed 35 participants on the MAS and MTS twice, first for the test and second a week later for the retest. Two observers performed the measurements. Feasibility was assessed based on the percentage of successful measurements. Test-retest and interrater reliability were determined by using the Wilcoxon signed rank test, intraclass correlation coefficients (ICC), Spearman's correlation, and either limits of agreement (LOA) or quadratically weighted kappa. The feasibility of the measurements was good, because an acceptable percentage of successful measurements were performed. MAS measurements had substantial to almost perfect quadratically weighted kappa (>0.8) and an acceptable ICC (>0.8) for both inter- and intrarater reliability. However, MTS measurements had insufficient ICCs, Spearman's correlations, and LOAs for both inter- and interrater reliability. Our data indicated that the feasibility of the MAS and MTS for measuring muscle tone in persons with PIMD was good. The MAS had sufficient test-retest and interrater reliability; however, the MTS had an insufficient test-retest and interrater reliability in persons with PIMD. Thus, the MAS may be a good method for evaluating the quality of daily movements in persons with PIMD. Providing test administrators with training and clear instructions will improve test reliability.

Background The purpose of this study is to describe feasibility and test–retest reliability of the six-minute walking distance test (6MWD) and an adapted shuttle run test (aSRT) in persons with severe intellectual and sensory (multiple) disabilities. Materials and Methods Forty-seven persons with severe multiple disabilities, with Gross Motor Function Classification System (GMFCS) grade I and II and wearing a heart rate monitor, performed the 6MWD and the aSRT twice. Results Ninety-six per cent of the participants completed both tests successfully. Wilcoxon signed-rank test revealed no significant differences between test and retest (P < 0.05). Intraclass correlation coefficients for all variables were ≥0.90. Limits of agreement for aSRT in GMFCS II subjects were insufficient. Conclusion Six-minute walking distance test is feasible and reliable for measuring functional exercise capacity in GMFCS I and II participants with severe multiple disabilities. aSRT is feasible and reliable for measuring aerobic capacity in GMFCS I participants. Compared with others, participants with severe multiple disabilities achieved poor results in 6MWD.

Talent development programmes for professional soccer clubs aim to guide players towards professional level performance. We identify whether the intermittent endurance capacity of these players may have changed over time. Since the 2000/2001 competition season, the intermittent endurance capacity of players in the talent development programmes of two professional soccer clubs was measured annually. A total of 492 players participated, divided across seven age categories (under 13 (U13), U14, U15, U16, U17, U18, U19) and resulting in 953 measurements. Analyses of variance showed an improvement in intermittent endurance capacity from the 2000/2001 season to 2009/2010 of around 50% in all age groups (P < 0.05). A possible explanation is the increased quantity and quality of training over the years. When identifying, developing and selecting young players, scouts, trainers and coaches have to be aware that the current level of soccer and its underlying performance characteristics--such as intermittent endurance capacity--are improving over time. This factor may have consequences for current young players aiming to make it to the top 10 years from now.
