Assistive technology supports maintenance or improvement of an individual’s functioning and independence, though for people in need the access to assistive products is not always guaranteed. This paper presents a generic quality framework for assistive technology service delivery that can be used independent of the setting, context, legislative framework, or type of technology. Based on available literature and a series of discussions among the authors, a framework was developed. It consists of 7 general quality criteria and four indicators for each of these criteria. The criteria are: accessibility; competence; coordination; efficiency; flexibility; user centeredness, and infrastructure. This framework can be used at a micro level (processes around individual users), meso level (the service delivery scheme or programme) or at a macro level (the whole country). It aims to help identify in an easy way the main strengths and weaknesses of a system or process, and thus guide possible improvements. As a next step in the development of this quality framework the authors propose to organise a global consultancy process to obtain responses from stakeholders across the world and to plan a number of case studies in which the framework is applied to different service delivery systems and processes in different countries.

Technological innovations such as parcel lockers can only contribute to reducing the negative impact of last mile logistics on people, planet and profit outcomes if consumers adopt them. Using the Unified Theory of Acceptance and Use of Technology (UTAUT) as a theoretical lens, the purpose of this study was to design effective interventions in the pre-delivery stage that stimulated consumers to choose to use parcel lockers, focusing on incentives and actions that online retailers and logistic service providers can easily implement. A vignette-based experiment was used to examine the impact of providing consumers with information about sustainability, information about parcel lockers and information about distance to nearest parcel locker on their choice to use a parcel locker instead of receiving their package at home. The originality of the study is twofold. The role of geographical context which is oftentimes overlooked was examined explicitly in this study by conducting the study with participants from the Netherlands and Lithuania. The present study used causal mediation analysis to study the combined effects of interventions and previous experience with parcel lockers through the UTAUT constructs. The findings showed that providing information about sustainability and information about the distance to the nearest parcel locker were effective interventions, however the effect of distance was country specific. Causal mediation analysis showed that familiarity with parcel lockers was positively associated with the choice to use parcel lockers by increasing perceived ease of use, expected performance, and reducing perceived risk.

Companies in the Brainport region are often characterized as high mix low volume (HMLV) production environments. These companies are distinguished by a wide range of possible products (high product variety), which are produced in low volumes. These are often customer-specific products that are produced once or incidentally. Traditionally, these companies focus on efficient use of resources, where utilisation rate and cost coverage are relevant. The increasing customer demand in the region leads to pressure on production capacity. An initial intuitive response from these companies is to further increase the utilisation rate of machines. To keep costs manageable, the company tries to avoid investing in additional capacity. An undesirable side effect is increasing pressure on timeliness (delivery, such as lead times, delivery reliability, flexibility) and quality. The apparent contradiction between costs and timeliness in these HMLV production environments is a recurring issue in practice-oriented research conducted by Fontys Industrial Engineering and Management students. This results in the following research question: Which sub-aspects may be relevant to the performance regarding Quality, Delivery, and Cost (QDC) of an HMLV production environment?

Background: Post-term pregnancy, a pregnancy exceeding 294 days or 42 completed weeks, is associated with increased perinatal morbidity and mortality and is considered a high-risk condition which requires specialist surveillance and induction of labour. However, there is uncertainty on the policy concerning the timing of induction for post-term pregnancy or impending post-term pregnancy, leading to practice variation between caregivers. Previous studies on induction at or beyond 41 weeks versus expectant management showed different results on perinatal outcome though conclusions in meta-analyses show a preference for induction at 41 weeks. However, interpretation of the results is hampered by the limited sample size of most trials and the heterogeneity in design. Most control groups had a policy of awaiting spontaneous onset of labour that went far beyond 42 weeks, which does not reflect usual care in The Netherlands where induction of labour at 42 weeks is the regular policy. Thus leaving the question unanswered if induction at 41 weeks results in better perinatal outcomes than expectant management until 42 weeks. Methods/design: In this study we compare a policy of labour induction at 41 + 0/+1 weeks with a policy of expectant management until 42 weeks in obstetrical low risk women without contra-indications for expectant management until 42 weeks and a singleton pregnancy in cephalic position. We will perform a multicenter randomised controlled clinical trial. Our primary outcome will be a composite outcome of perinatal mortality and neonatal morbidity. Secondary outcomes will be maternal outcomes as mode of delivery (operative vaginal delivery and Caesarean section), need for analgesia and postpartum haemorrhage (≥1000 ml). Maternal preferences, satisfaction, wellbeing, pain and anxiety will be assessed alongside the trial. Discussion: his study will provide evidence for the management of pregnant women reaching a gestational age of 41 weeks.

Rationale, aims and objective: Primary Care Plus (PC+) focuses on the substitution of hospital-based medical care to the primary care setting without moving hospital facilities. The aim of this study was to examine whether population health and experience of care in PC+ could be maintained. Therefore, health-related quality of life (HRQoL) and experienced quality of care from a patient perspective were compared between patients referred to PC+ and to hospital-based outpatient care (HBOC). Methods: This cohort study included patients from a Dutch region, visiting PC+ or HBOC between December 2014 and April 2018. With patient questionnaires (T0, T1 and T2), the HRQoL and experience of care were measured. One-to-two nearest neighbour calliper propensity score matching (PSM) was used to control for potential selection bias. Outcomes were compared using marginal linear models and Pearson chi-square tests. Results: One thousand one hundred thirteen PC+ patients were matched to 606 HBOC patients with well-balanced baseline characteristics (SMDs <0.1). Regarding HRQoL outcomes, no significant interaction terms between time and group were found (P > .05), indicating no difference in HRQoL development between the groups over time. Regarding experienced quality of care, no differences were found between PC+ and HBOC patients. Only travel time was significantly shorter in the HBOC group (P ≤ .001). Conclusion: Results show equal effects on HRQoL outcomes over time between the groups. Regarding experienced quality of care, only differences in travel time were found. Taken as a whole, population health and quality of care were maintained with PC+ and future research should focus more on cost-related outcomes.

Purpose: This is a position paper describing the elements of an international framework for assistive techhnology provision that could guide the development of policies, systems and service delivery procedures across the world. It describes general requirements, quality criteria and possible approaches that may help to enhance the accessibility of affordable and high quality assistive technology solutions. Materials and methods: The paper is based on the experience of the authors, an analysis of the existing literature and the inputs from many colleagues in the field of assistive technology provision. It includes the results of discussions of an earlier version of the paper during an international conference on the topic in August 2017. Results and conclusion: The paper ends with the recommendation to develop an international standard for assistive technology provision. Such a standard can have a major impact on the accessibility of AT for people with disabilities. The paper outlines some the key elements to be included in a standard.

Nationwide and across the globe, the quality, affordability, and accessibility of home-based healthcare are under pressure. This issue stems from two main factors: the rapidly growing ageing population and the concurrent scarcity of healthcare professionals. Older people aspire to live independently in their homes for as long as possible. Additionally, governments worldwide have embraced policies promoting “ageing in place,” reallocating resources from institutions to homes and prioritising home-based services to honour the desire of older people to continue living at home while simultaneously addressing the rising costs associated with traditional institutional care.Considering the vital role of district nursing care and the fact that the population of older people in need of assistance at home is growing, it becomes clear that district nursing care plays a crucial role in primary care. The aim of this thesis is twofold: 1) to strengthen the evidence base for district nursing care; and 2) to explore the use of outcomes for learning and improving in district nursing care. The first part of this thesis examines the current delivery of district nursing care and explores its challenges during the COVID-19 pandemic to strengthen the evidence base and get a better understanding of district nursing care. Alongside the goal of strengthening the evidence for district nursing care, the second part of this thesis explores the use of patient outcomes for learning and improving district nursing care. It focuses on nurse-sensitive patient outcomes relevant to district nursing care, their current measurement in practice, and what is needed to use outcomes for learning and improving district nursing practice.

Background: Blended face-to-face and web-based treatment is a promising mode to deliver smoking cessation treatment. In an outpatient clinic in a Dutch Hospital effectiveness of a blended treatment (BSCT) was compared to usual face-to-face treatment (F2F). The results from 6 months post-treatment follow-up are presented here.Methods: In this open-label two-arm non-inferiority RCT patients (N=344) of a Dutch outpatient smoking cessation clinic were assigned either to the blended smoking cessation treatment (BSCT, N=167) or a face-to-face treatment with identical ingredients and duration (F2F, N=177). CO-validated point prevalence abstinence at 6 months follow-up, taken shortly after end of treatment was analyzed. Intention-to-treat analyses were performed, retaining missing participants as continuing smokers. Non-inferiority was assessed based on a one-sided margin of five percentage points difference between arms. Additionally, a Bayes Factor was estimated (with a BF>3 supporting non-inferiority, and a <.3 rejecting non-inferiority).Method: At 6 months follow up, 23 BSCT participants (13.8%) and 31 F2F participants (17.5%) were abstinent, with a difference of 3.7% (95%CI: 11.4;-4.0) in favor of F2F. Furthermore, a BF=1.28 was found.Discussion: Based on observed biochemically validated abstinence rates, this RCT suggests that delivering outpatient smoking cessation treatment in a blended mode yields comparable quit rates as full face-to-face treatment mode. However, non-inferiority could not be supported conclusively. Ignoring patient preferences for either of the delivery modes may explain these inconclusive findings.

As artificial intelligence (AI) reshapes hiring, organizations increasingly rely on AI-enhanced selection methods such as chatbot-led interviews and algorithmic resume screening. While AI offers efficiency and scalability, concerns persist regarding fairness, transparency, and trust. This qualitative study applies the Artificially Intelligent Device Use Acceptance (AIDUA) model to examine how job applicants perceive and respond to AI-driven hiring. Drawing on semi-structured interviews with 15 professionals, the study explores how social influence, anthropomorphism, and performance expectancy shape applicant acceptance, while concerns about transparency and fairness emerge as key barriers. Participants expressed a strong preference for hybrid AI-human hiring models, emphasizing the importance of explainability and human oversight. The study refines the AIDUA model in the recruitment context and offers practical recommendations for organizations seeking to implement AI ethically and effectively in selection processes.

Objective: The aim of this study was to obtain insight in specific elements influencing the use, non-use, satisfaction, and dissatisfaction of ankle foot orthoses (AFOs) and the presence of underexposed problems with respect to AFOs. Methods: A questionnaire was composed to obtain information from AFO users to investigate the variables associated with satisfaction and the relation between these variables. A specific feature of this study was the systematic analysis of the remarks made by the respondents about their AFO. Quantitative data analyses were used for analysing the satisfaction and qualitative analyses were used analysing the remarks of the respondents. A total of 211 users completed the questionnaire. Results: Our survey showed that 1 out of 15 AFOs were not used at all. About three quarters of the AFO users were satisfied and about one quarter was dissatisfied. Females and users living alone reported relatively high levels of dissatisfaction, especially in the field of dimensions, comfort, weight, safety and effectiveness. Dissatisfaction with respect to off-the-shelf AFOs for the item durability was higher than that for custom-made AFOs. In the delivery and maintenance process the items ‘maintenance’, ‘professionalism’ and ‘delivery follow-up’ were judged to be unsatisfactory. A large number of comments were made by the respondents to improve the device or process, mainly by the satisfied AFO users. These comments show that even satisfied users experience many problems and that a lot of problems of AFO users are ‘underexposed’. Conclusion: To improve user satisfaction, the user practice has to be identified as an important sub-process of the whole orthopaedic chain especially in the diagnosis and prescription, delivery tuning and maintenance, and evaluation phase.
