The objective of this study was to determine the feasibility and efficacy of an exercise training program to improve exercise capacity and fatigue level in pediatric patients with end-stage renal disease (ESRD). Twenty children on dialysis intended to perform a 12-week graded community-based exercise program. Exercise capacity and fatigue level were studied; muscle force and health-related quality of life were secondary outcomes. All outcomes were measured at baseline (T = 0) and after intervention (T = 1). Fourteen of the 20 patients (70%) either did not start the program or did not complete the program. Of these patients, seven did not complete or even start the exercise program because of a combination of lack of time and motivational problems. Six patients were not able to continue the program or were unable to do the follow-up measurements because of medical problems. Exercise capacity and muscle strength was higher after the exercise program in the children who completed the training. In conclusion, exercise training is difficult to perform in children with ESRD and is not always feasible in real-life situations for many children with ESRD.

PurposeThis study evaluated current fertility care forCKD patients by assessing the perspectives of nephrolo-gists and nurses in the dialysis department.MethodsTwo different surveys were distributed forthis cross-sectional study among Dutch nephrologists(N=312) and dialysis nurses (N=1211). ResultsResponse rates were 50.9% (nephrologists) and45.4% (nurses). Guidelines on fertility care were presentin the departments of 9.0% of the nephrologists and 15.6%of the nurses. 61.7% of the nephrologists and 23.6% ofthe nurses informed ≥50% of their patients on potentialchanges in fertility due to a decline in renal function.Fertility subjects discussed by nephrologists included “wishto have children” (91.2%), “risk of pregnancy for patients’health” (85.8%), and “inheritance of the disease” (81.4%).Barriers withholding nurses from discussing FD werebased on “the age of the patient” (62.6%), “insufficienttraining” (55.2%), and “language and ethnicity” (51.6%).29.2% of the nurses felt competent in discussing fertility,8.3% had sufficient knowledge about fertility, and 75.7%needed to expand their knowledge. More knowledge andcompetence were associated with providing fertility healthcare (p< 0.01). ConclusionsIn most nephrology departments, the guide-lines to appoint which care provider should provide fertil-ity care to CKD patients are absent. Fertility counselingis routinely provided by most nephrologists, nurses oftenskip this part of care mainly due to insufficiencies in self-imposed competence and knowledge and barriers based oncultural diversity. The outcomes identified a need for fer-tility guidelines in the nephrology department and trainingand education for nurses on providing fertility care. Objective: Dialysis patients frequently report a change of taste that is reversible after renal transplantation, suggesting that uremic toxins may negatively influence taste. Currently, frequent nocturnal home hemodialysis (NHHD) is the most effective method of hemodialysis, and is associated with the lowest levels of uremic toxins. We studied preferences for various foods as an indicator of taste perception. We questioned whether food preference differs between NHHD patients and those on conventional hemodialysis. Design and Patients: In this transverse, cross-sectional pilot study, we assessed food preference by means of a questionnaire for patients on NHHD (n = 6; 8 hours of dialysis per night, for 5 or 6 nights a week) and 3 age-matched and sex-matched control groups: chronic home hemodialysis patients (HHD; n = 9; 4 to 5 hours of dialysis per day, 3 days a week), chronic in-center hemodialysis patients (CHD; n = 18; 4 to 5 hours of dialysis per day, 3 days a week), and healthy control subjects (HC; n = 23). Results: Mean scores for food preference did not differ between groups (P = .32). Similarly, the preference for product groups did not differ between groups. On an individual product level, we found only minor differences. The NHHD patients had a preference for savory snacks, as did the HC and CHD groups, whereas the HHD group had a preference for sweet snacks (P < .05). Hemodialysis patients reported dry mouth more often than did the HC patients (P < .05). Conclusions: Frequent NHHD has no major impact on food preference. The change in taste reported by NHHD patients is not related to their particular food preferences. © 2010 National Kidney Foundation, Inc.
