The paper presents a framework that through structured analysis of accident reports explores the differences between practice and academic literature as well amongst organizations regarding their views on human error. The framework is based on the hypothesis that the wording of accident reports reflects the safety thinking and models that have been applied during the investigation, and includes 10 aspects identified in the state-of-the-art literature. The framework was applied to 52 air accident reports published by the Dutch Safety Board (DSB) and 45 ones issued by the Australian Transport Safety Bureau (ATSB) from 1999 to 2014. Frequency analysis and statistical tests showed that the presence of the aspects in the accident reports varied from 32.6% to 81.7%, and revealed differences between the ATSB and the DSB approaches to human error. However, in overall safety thinking have not changed over time, thus, suggesting that academic propositions might have not yet affected practice dramatically.

Background To gain insight into the role of plantar intrinsic foot muscles in fall-related gait parameters in older adults, it is fundamental to assess foot muscles separately. Ultrasonography is considered a promising instrument to quantify the strength capacity of individual muscles by assessing their morphology. The main goal of this study was to investigate the intra-assessor reliability and measurement error for ultrasound measures for the morphology of selected foot muscles and the plantar fascia in older adults using a tablet-based device. The secondary aim was to compare the measurement error between older and younger adults and between two different ultrasound machines. Methods Ultrasound images of selected foot muscles and the plantar fascia were collected in younger and older adults by a single operator, intensively trained in scanning the foot muscles, on two occasions, 1–8 days apart, using a tablet-based and a mainframe system. The intra-assessor reliability and standard error of measurement for the cross-sectional area and/or thickness were assessed by analysis of variance. The error variance was statistically compared across age groups and machines. Results Eighteen physically active older adults (mean age 73.8 (SD: 4.9) years) and ten younger adults (mean age 21.9 (SD: 1.8) years) participated in the study. In older adults, the standard error of measurement ranged from 2.8 to 11.9%. The ICC ranged from 0.57 to 0.97, but was excellent in most cases. The error variance for six morphology measures was statistically smaller in younger adults, but was small in older adults as well. When different error variances were observed across machines, overall, the tablet-based device showed superior repeatability. Conclusions This intra-assessor reliability study showed that a tablet-based ultrasound machine can be reliably used to assess the morphology of selected foot muscles in older adults, with the exception of plantar fascia thickness. Although the measurement errors were sometimes smaller in younger adults, they seem adequate in older adults to detect group mean hypertrophy as a response to training. A tablet-based ultrasound device seems to be a reliable alternative to a mainframe system. This advocates its use when foot muscle morphology in older adults is of interest.

The assumptions of the model for factor analysis do not exclude a class of indeterminate covariances between factors and error variables (Grayson, 2003). The construction of all factors of the model for factor analysis is generalized to incorporate indeterminate factor-error covariances. A necessary and sufficient condition is given for indeterminate factor-error covariances to be arbitrarily small, for mean square convergence of the regression predictor of factor scores, and for the existence of a unique determinate factor and error variable. The determinate factor and error variable are uncorrelated and satisfy the defining assumptions of factor analysis. Several examples are given to illustrate the results.

Abstract In this paper several meaningful audio icons of classic arcade games such as Pong, Donkey Kong, Mario World and Pac-Man are analyzed, using the PRAAT software for speech analysis and musical theory. The analysis results are used to describe how these examples of best practice sound design obtain their meaning in the player's perception. Some aspects can be related to the use of tonal hierarchy (e.g. Donkey Kong and Mario World) which is a western culture related aspect of musical meaning. Other aspects are related to universal expressions of meaning such as the theory of misattribution, prosody, vocalization and cross-modal perceptions such as brightness and the uncanny valley hypothesis. Recent studies in the field of cognitive neuroscience support the universal and meaningful potential of all these aspects. The relationship between language related prosody, vocalization and phonology, and music seems to be an especially successful design principle for universally meaningful music icons in game sound design.

A construction method is given for all factors that satisfy the assumptions of the model for factor analysis, including partially determined factors where certain error variances are zero. Various criteria for the seriousness of indeterminacy are related. It is shown that B. F. Green's (1976) conjecture holds: For a linear factor predictor the mean squared error of prediction is constant over all possible factors. A simple and general geometric interpretation of factor indeterminacy is given on the basis of the distance between multiple factors. It is illustrated that variable elimination can have a large effect on the seriousness of factor indeterminacy. A simulation study reveals that if the mean square error of factor prediction equals .5, then two thirds of the persons are "correctly" selected by the best linear factor predictor. The purpose of this paper is to perform a metaphorical analysis of knowledge as energy. This paper is based on a theoretical research concerning the nature, perception, basic laws and challenges brought up by these fundamental concepts of knowledge and energy. The metaphorical analysis of knowledge and intellectual capital has been initiated by Daniel Andriessen and his findings have been presented in several seminal works (Andriessen, 2006; 2008; Andriessen and Boom, 2007). In his work, Andriessen concluded we need to find new metaphors for knowledge. In our theoretical research we shall consider the knowledge as energy metaphor, with energy as the source domain, and knowledge as the target domain, and we are interested in identifying the metaphorical semantic kernel and the limitations of this analysis. The semantic kernel contains: (1) the concept of field as a nonuniform and nonlinear distribution of knowledge; (2) dynamics of potential and kinetic forms of manifestations; (3) dynamics of work and heat, and (4) entropy and syntropy process characteristics. Limitations of this analysis come from the conservation laws of energy transformation which cannot be applied to the knowledge domain.

Background: The aim of this study is to validate a newly developed nurses' self-efficacy sources inventory. We test the validity of a five-dimensional model of sources of self-efficacy, which we contrast with the traditional four-dimensional model based on Bandura's theoretical concepts. Methods: Confirmatory factor analysis was used in the development of the newly developed self-efficacy measure. Model fit was evaluated based upon commonly recommended goodness-of-fit indices, including the χ2 of the model fit, the Root Mean Square Error of approximation (RMSEA), the Tucker-Lewis Index (TLI), the Standardized Root Mean Square Residual (SRMR), and the Bayesian Information Criterion (BIC). Results: All 22 items of the newly developed five-factor sources of self-efficacy have high factor loadings (range .40-.80). Structural equation modeling showed that a five-factor model is favoured over the four-factor model. Conclusions and implications: Results of this study show that differentiation of the vicarious experience source into a peer- and expert based source reflects better how nursing students develop self-efficacy beliefs. This has implications for clinical learning environments: a better and differentiated use of self-efficacy sources can stimulate the professional development of nursing students.

AbstractIn many biomechanical motion studies, kinematic parameters are estimated from position measurements on a number of landmarks. In the present investigation, dummy motion experiments are performed in order to study the error dependence of kinematic parameters on geometric factors (number of markers, isotropic vs anisotropic landmark distributions, landmark distribution size), on kinematic factors (rotation step magnitude, the presence of translational displacements, the distance of the landmarks' mean position to the rotation axis), and on anisotropically distributed measurement errors. The experimental results are compared with theoretical predictions of a previous error analysis assuming isotropic conditions for the measurement errors and for the spatial landmark distribution. In general, the experimental findings agree with the predictions of the error model. The kinematic parameters such as translations and rotations are well-determined by the model. In the helical motion description, the same applies for the finite rotation angle about and the finite shift along the helical axis. However, the direction and position of the helical axis are ill-determined. An anisotropic landmark distribution with relatively few markers located in the direction of the rotation axis will even aggravate the ill-posed nature of the finite helical axis estimation.

We conducted a descriptive study among first-year engineering students at the Anton de Kom University of Suriname. We analyzed students’ errors regarding necessary prior knowledge in a calculus A exam. We found that the stage of the solution in which prior knowledge is required impacts the importance of prior knowledge. We also found that many errors concerned basic algebra and trigonometry concepts and skills. We concluded that even though the required prior knowledge concerns basic algebra and trigonometry, the stage of the solution in which prior knowledge is needed is of great importance.

Medical imaging practice changed dramatically with the introduction of digital imaging. Although digital imaging has many advantages, it also has made it easier to delete images that are not of diagnostic quality. Mistakes in imaging—from improper patient positioning, patient movement during the examination, and selecting improper equipment—could go undetected when images are deleted. Such an approach would preclude a reject analysis from which valuable lessons could be learned. In the analog days of radiography, saving the rejected films and then analyzing them was common practice among radiographers. In principle, reject analysis can be carried out easier and with better tools (ie, software) in the digital era, provided that rejected images are stored for analysis. Reject analysis and the subsequent lessons learned could reduce the number of repeat images, thus reducing imaging costs and decreasing patient exposure to radiation. The purpose of this study, which was conducted by order of the Dutch Healthcare Inspectorate, was to investigate whether hospitals in the Netherlands store and analyze failed imaging and, if so, to identify the tools used to analyze those images.
