Furosemide is included in the World Anti-Doping Agency’s (WADA) list of prohibited substances because it can be used by athletes to mask the presence of performance-enhancing drugs in urine and/or excrete water for rapid weight loss. But how effective is furosemide in masking prohibited substances in urine? Based on the pharmacology and the available literature, we conclude that the masking effect of furosemide is limited. Furosemide is a doping agent that is mainly relevant for sports with weight categories. Conflict of interest and financial support: none declared.

Disease-related undernutrition is highly prevalent and requires timely intervention. However, identifying undernutrition often relies on physician judgment. As Internal Medicine wards are the backbone of the hospital setting, insight into the prevalence of nutritional risk in this population is essential. We aimed to determine the prevalence of nutritional risk in Internal Medicine wards, to identify its correlates, and to assess the agreement between the physicians’ impression of nutritional risk and evaluation by Nutritional Risk Screening 2002.

To identify ethical issues that interns encounter in their clinical education and thus build a more empirical basis for the required contents of the clinical ethics curriculum. The authors analyzed a total of 522 required case reports on ethical dilemmas experienced by interns from September 1995 to May 1999 at the medical school of Vrije Universiteit in Amsterdam. They identified four regularly described and numerous less frequently described topics. The interns addressed a wide range of ethical themes. In 45% of the cases, they mentioned disclosure or non-disclosure of information and informed consent; in 37%, medical decisions at the end of life; in 16%, medical failures; and in 9%, problems transferring patients from one caregiver to another. The interns also identified 27 themes linked to their unique position as interns and 19 themes related to specific types of patients. Based on self-reported experiences, the authors conclude that clinical ethics teachers should reflect on a multitude of dilemmas. Special expertise is required with respect to end-of-life decisions, truth telling, medical failures, and transferring patients from one caregiver to another. The clinical ethics curriculum should encourage students to voice their opinions and deal with values, responsibilities, and the uncertainty and failings of medical interventions.

Kijkend naar de ontwikkelingen in de medische en farmaceutische zorg, concludeer ik dat het belang van innovaties niet altijd in overeenstemming is met de snelheid waarmee die innovaties hun plek krijgen in het standaardhandelingsarsenaal van zorgverleners. Veranderingen in de zorg gaan vaak langzaam en doorbraken worden slecht herkend. De vraag is hoe dit komt. Er blijken vele factoren van invloed op het mogelijke succes van een innovatie. Van groot belang is het inzicht dat innoveren meer is dan iets bedenken en dan maar aannemen dat het wel zal worden opgepikt door de (potentiële) doelgroep. Het aan de man brengen (‘dissemineren’) van de innovatie is mede bepalend voor een succesvolle implementatie. In de farmaceutische zorg is voor deze overbruggingsfunctie een belangrijke rol weggelegd voor de farmakundige. Mijn lectoraat, dat is gekoppeld aan de opleiding Farmakunde, zal zich bezighouden met het onderzoek naar het proces om farmaceutische innovaties te dissemineren. In deze openbare les licht ik de context en consequenties van dit onderzoeksthema nader toe. Ik begin met een uitleg van de farmakundige en diens toegevoegde waarde in het werkveld (hoofdstuk 1), en vervolg met een korte beschrijving van recente veranderingen binnen de zorg (hoofdstuk 2). In het begeleiden van die veranderingen ligt een belangrijke meerwaarde van de farmakundige, en de missie van dit lectoraat. Daarna (hoofdstuk 3) beschouwen we het innoveren in de (farmaceutische) gezondheidszorg in meer detail. Hoofdstuk 4 geeft diverse handvatten voor het kiezen van de juiste interventies om de afstand tussen de innovator en de toekomstige gebruiker te overbruggen en zodoende de toegang voor de gebruiker tot de innovatie te verbeteren. Background Interprofessional education is promoted as a means of enhancing future collaborative practice in healthcare. We developed a learning activity in which undergraduate medical, nursing and allied healthcare students practice interprofessional collaboration during a student-led interprofessional team meeting. Design and delivery During their clinical rotation at a family physician’s practice, each medical student visits a frail elderly patient and prepares a care plan for the patient. At a student-led interprofessional team meeting, medical, nursing and allied healthcare students jointly review these care plans. Subsequently, participating students reflect on their interprofessional collaboration during the team meeting, both collectively and individually. Every 4 weeks, six interprofessional team meetings take place. Each team comprises 9–10 students from various healthcare professions, and meets once. To date an average of 360 medical and 360 nursing and allied healthcare students have participated in this course annually. Evaluation Students mostly reported positive experiences, including the opportunity to learn with, from and about other healthcare professions in the course of jointly reviewing care plans, and feeling collectively responsible for the care of the patients involved. Additionally, students reported a better understanding of the contextual factors at hand. The variety of patient cases, diversity of participating health professions, and the course material need improvement. Conclusion Students from participating institutions confirmed that attending a student-led interprofessional team meeting had enabled them to learn with, from and about other health professions in an active role. The use of real-life cases and the educational design contributed to the positive outcome of this interprofessional learning activity.

Designing authentic, project-based learning environments in higher professional education is far from clear-cut yet and can be a difficult task for teachers. The research question driving our research was: How can we design and improve project-based, ICT-supported learning environments in higher professional education? It was our aim to find valuable pieces of the design-puzzle in current literature and integrate these pieces. We intended to complement current insights with inventive insights from an explicit design perspective by carrying out empirical studies.
