Background: Only a few papers are published on the safety and effectiveness of acute burn care in low-income countries. A cohort study was therefore carried out to determine such outcomes.Methods: The study was conducted in a rural Tanzanian hospital in 2017-2018. All patients admitted with burns were eligible. Complications were scored during admission as an indication for safety. Survivors of severe burn injuries were evaluated for time of reepithelialization, graft take, disability (WHODAS2.0) and quality of life (EQ5D-3L) up to 3 months post-injury, as an indication of effectiveness.Results: Patients presented on average at 5 days after injury (SD 11, median 1, IQR 0-4). Three patients died at admission. The remaining 79 were included in the cohort. Their median age was 3 years (IQR 2-9, range 0.5-49), mean TBSA burned 12% (SD10%) and mortality rate 11.4%. No surgery-related mortality or life-threatening complications were observed. Skin grafting was performed on 29 patients at a delayed stage (median 23 days, IQR 15-47). Complications of skin grafts included partial (25% of procedures) and complete graft necrosis (8% of procedures). The mean time to reepithelialization was 52 (SD 42) days after admission. Disability and quality of life improved from admission to 3 months after injury (p<0.001, p<0.001, respectively).Conclusion: In this resource-limited setting patients presented after a delay and with multiple complications. The mortality during the first two weeks after admission was high. Surgery was found to be safe and effective. A significant improvement in disability and quality of life was observed.

This work is on 3-D localization of sensor motes in massive swarms based solely on 1-D relative distance-measurements between neighbouring motes. We target applications in remote and difficult-to-access environments such as the exploration and mapping of the interior of oil reservoirs where hundreds or thousands of motes are used. These applications bring forward the need to use highly miniaturized sensor motes of less than 1 centimeter, thereby significantly limiting measurement and processing capabilities. These constraints, in combination with additional limitations posed by the environments, impede the communication of unique hardware identifiers, as well as communication with external, fixed beacons.

This research focuses on exit choices within SMEs. In this study, “exit choice” refers to the decision to opt for either liquidation or sale of the firm. The predictions focus on human-capital and firm-resource variables. The hypotheses are tested on a set of 158 owners of small firms, the majority of which are micro-firms with 0–9 employees. The results of a series of binominal logistic regression analyses show that firm-resource characteristics (previous sales turnover, the firm’s independence from its owner, and firm size), together with one aspect of the owner’s specific human capital (the owner’s acquisition experience), predict exit choice. The conclusions have been made with caution, as the dataset is relatively small and the number of predictors is limited.

Neighborhood image processing operations on Field Programmable Gate Array (FPGA) are considered as memory intensive operations. A large memory bandwidth is required to transfer the required pixel data from external memory to the processing unit. On-chip image buffers are employed to reduce this data transfer rate. Conventional image buffers, implemented either by using FPGA logic resources or embedded memories are resource inefficient. They exhaust the limited FPGA resources quickly. Consequently, hardware implementation of neighborhood operations becomes expensive, and integrating them in resource constrained devices becomes unfeasible. This paper presents a resource efficient FPGA based on-chip buffer architecture. The proposed architecture utilizes full capacity of a single Xilinx BlockRAM (BRAM36 primitive) for storing multiple rows of input image. To get multiple pixels/clock in a user defined scan order, an efficient duty-cycle based memory accessing technique is coupled with a customized addressing circuitry. This accessing technique exploits switching capabilities of BRAM to read 4 pixels in a single clock cycle without degrading system frequency. The addressing circuitry provides multiple pixels/clock in any user defined scan order to implement a wide range of neighborhood operations. With the saving of 83% BRAM resources, the buffer architecture operates at 278 MHz on Xilinx Artix-7 FPGA with an efficiency of 1.3 clock/pixel. It is thus capable to fulfill real time image processing requirements for HD image resolution (1080 × 1920) @103 fcps.

The increasing demand for Prunus africana resources is an opportunity for its conservation and commercial use to support livelihoods in Africa. The objective for this study was to investigate major steps to advance production of P. africana for long-term commercial use in Uganda. Specific objectives were to explore potential production schemes, setbacks in production and strategies to advance it. The study was done by review of literature, documents and interviews with experts. Results indicated Agroforestry and large plantations to be useful schemes for production. Identified setbacks are: low trade in P. africana, unknown returns from production, competing land uses, long growth period, limited market assurance and information. The lack of a resource assessment for P. africana in forests contributes to its low trade which undermines related economic benefits for national development and incentives to commercial production. We propose that a national Quantitative resource assessment of P. africana in forests is one of the crucial steps that should be undertaken to carefully organise and advance sustainable trade to provide rational incentives for commercial production. Subsequently, production should be localised in suitable sites and producers be organised into cooperatives. Further research to improve returns from commercial production of P. africana is needed.

BACKGROUND: The SpO2/FiO2 is a useful oxygenation parameter with prognostic capacity in patients with ARDS. We investigated the prognostic capacity of SpO2/FiO2 for mortality in patients with ARDS due to COVID-19.METHODS: This was a post-hoc analysis of a national multicenter cohort study in invasively ventilated patients with ARDS due to COVID-19. The primary endpoint was 28-day mortality.RESULTS: In 869 invasively ventilated patients, 28-day mortality was 30.1%. The SpO2/FiO2 on day 1 had no prognostic value. The SpO2/FiO2 on day 2 and day 3 had prognostic capacity for death, with the best cut-offs being 179 and 199, respectively. Both SpO2/FiO2 on day 2 (OR, 0.66 [95%-CI 0.46-0.96]) and on day 3 (OR, 0.70 [95%-CI 0.51-0.96]) were associated with 28-day mortality in a model corrected for age, pH, lactate levels and kidney dysfunction (AUROC 0.78 [0.76-0.79]). The measured PaO2/FiO2 and the PaO2/FiO2 calculated from SpO2/FiO2 were strongly correlated (Spearman's r = 0.79).CONCLUSIONS: In this cohort of patients with ARDS due to COVID-19, the SpO2/FiO2 on day 2 and day 3 are independently associated with and have prognostic capacity for 28-day mortality. The SpO2/FiO2 is a useful metric for risk stratification in invasively ventilated COVID-19 patients.

With growing environmental concerns, upcycling has become an important theme in literature and practice. Upcycling can help slow and close resource cycles through product life-extension. Cities offer opportunities for upcycling initiatives and seek to tackle challenges in urban solid waste management by encouraging entrepreneurs to create value from local waste streams in urban resource centres and circular crafts centres. However, little is known about what drives urban upcycling and which barriers and drivers occur. This study explores urban upcycling in the context of the Dutch furniture industry, since The Netherlands positions itself as a ‘circular economy hotspot’ and furniture offers promising opportunities and best practices for upcycling. The analysis of 29 semi-structured interviews with experts engaged in urban upcycling reveals personal motives, drivers and barriers. Personal motives include (1) a personal purpose to ‘do good’, (2) an urge to challenge the status quo and (3) learning and inspiring by doing. Key drivers entail opportunities to (1) engage in collaborative experimentation, (2) participate in cross-sectoral local networks, (3) develop resource-based adaptive competences, (4) respond to increasing demand for upcycled products and (5) make social business activities financially viable. Key barriers perceived by upcycling experts include (1) limitations in resource availability, (2) increasing capacity requirements, (3) negative public quality perception, (4) limited marketing competences and (5) an unequal playing field. This study contributes with a comprehensive definition of urban upcycling and a structured overview of key factors that drive and constrain urban upcycling.

Anomaly detection is a key factor in the processing of large amounts of sensor data from Wireless Sensor Networks (WSN). Efficient anomaly detection algorithms can be devised performing online node-local computations and reducing communication overhead, thus improving the use of the limited hardware resources. This work introduces a fixed-point embedded implementation of Online Sequential Extreme Learning Machine (OS-ELM), an online learning algorithm for Single Layer Feed forward Neural Networks (SLFN). To overcome the stability issues introduced by the fixed precision, we apply correction mechanisms previously proposed for Recursive Least Squares (RLS). The proposed implementation is tested extensively with generated and real-world datasets, and compared with RLS, Linear Least Squares Estimation, and a rule-based method as benchmarks. The methods are evaluated on the prediction accuracy and on the detection of anomalies. The experimental results demonstrate that fixed-point OS-ELM can be successfully implemented on resource-limited embedded systems, with guarantees of numerical stability. Furthermore, the detection accuracy of fixed-point OS-ELM shows better generalization properties in comparison with, for instance, fixed-point RLS. © 2013 IEEE.

Objectives: The purpose of the current study was to assess the short-term effectiveness of short and intensive speech therapy provided to patients with cleft (lip and) palate (C(L)P) in terms of articulation and resonance. Methods: Five Ugandan patients (age: 7.3-19.6 years) with non-syndromic C(L)P received six hours of individualized speech therapy in three to four days. Speech therapy focused on correct phonetic placement and contrasts between oral and nasal airflow and resonance. Speech evaluations performed before and immediately after speech therapy, including perceptual and instrumental assessment techniques, were compared. Results: Post-therapy, improvement of speech was noted for most of the patients, although to varying degrees. Clinically relevant progress of objective nasalance values and/or articulation was obtained in four patients. Overall, two patients showed normal speech intelligibility, while three patients required additional speech therapy. Conclusion: These preliminary short-term results demonstrate that short and intensive speech therapy can be effective for patients with C(L)P in countries with limited access to speech-language therapy. However, further research is needed on the long-term effectiveness and the advantages of applying this treatment protocol in countries with good access to speech therapy. Learning outcomes: The reader will be able to (1) list the challenges in resource poorcountries to achieve access to speech-language therapy services, (2) describe when the application of speech therapy is appropriate in patients with C(L)P, (3) describe the speech therapy that can be applied to reduce compensatory articulation and resonance disorders in patients with C(L)P, and (4) list the (possible) advantages of short, intensive speech therapy for both resource-poor and developed countries.
