Linking Theory and Practice in Probation. Structured Decision Support for Case Management Plans

In this thesis several studies are presented that have targeted decision making about case management plans in probation. In a case management plan probation officers describe the goals and interventions that should help offenders stop reoffending, and the specific measures necessary to reduce acute risks of recidivism and harm. Such a plan is embedded in a judicial framework, a sanction or advice about the sanction in which these interventions and measures should be executed. The topic of this thesis is the use of structured decision support, and the question is if this can improve decision making about case management plans in probation and subsequently improve the effectiveness of offender supervision. In this chapter we first sketch why structured decision making was introduced in the Dutch probation services. Next we describe the instrument for risk and needs assessment as well as the procedure to develop case management plans that are used by the Dutch probation services and that are investigated in this thesis. Then we describe the setting of the studies and the research questions, and we conclude with an overview of this thesis.

Examining acculturation in three generations of Turkish-Dutch citizens

Despite decades of residence, Turkish-Dutch citizens, one of the largest immigrant groups in the Netherlands, continue to face significant disparities in health, social, and economic factors compared to native Dutch citizens. To better understand this persistent disparity, we examined the acculturation process of Turkish-Dutch citizens across three generations. Our study addressed two critical research gaps: (1) acculturation processes across three generations within a specific immigrant group, and (2) different acculturation domains across these generations. Data from 464 participants (232 Turkish-Dutch, 232 Dutch) show that acculturation varies significantly across generations (1st, 2nd, and 3rd) and domains (i.e., national identification, cultural values, language, and media use), with the second generation demonstrating the strongest resemblance to native Dutch citizens in most domains. These findings contribute to a nuanced understanding of acculturation processes and confirm the need for future research to consider generational differences and domain-specificity. The results have potential implications for policymakers and practitioners aiming to reduce disparities of Turkish-Dutch citizens with tailored policy and communication strategies.

Social work in the Netherlands is attracting an increasing number of Turkish and Moroccan Dutch professionals, mostly second-generation migrant women from a Muslim background. Inspired by Amartya Sen’s capability approach, this article presents the findings of a qualitative content analysis of 40 interviews with professionals by peers from the same background. The question is, what kind of professionals do these newly started social workers desire to be and what hindrances do they encounter? The professionals challenge the dominance of Western beliefs and values. This becomes tangible in their desires and constraints and especially in the process of choice.

Ethnic minority GP trainees at risk for underperformance assessments

Background: Recent studies suggest that ethnic minority students underperform in standardised assessments commonly used to evaluate their progress. This disparity seems to also hold for postgraduate medical students and GP trainees, and may affect the quality of primary health care, which requires an optimally diverse workforce. Aims: To address the following: 1) to determine to what extent ethnic minority GP trainees are more at risk of being assessed as underperforming than their majority peers; 2) to investigate whether established underperformance appears in specific competence areas; and 3) to explore first and second-generation ethnic minority trainees’ deviations. Design & setting: Quantitative retrospective cohort design in Dutch GP specialty training (start years: 2015–2017). Method: In 2020–2021, the authors evaluated files on assessed underperformance of 1700 GP trainees at seven Dutch GP specialty training institutes after excluding five opt-outs and 165 incomplete datasets (17.4% ethnic minority trainees). Underperformance was defined as the occurrence of the following, which was prompted by the training institute: 1) preliminary dropout; 2) extension of the educational pathway; and/or 3) mandatory coaching pathways. Statistics Netherlands (CBS) anonymised the files and added data about ethnic group. Thereafter, the authors performed logistic regression for potential underperformance analysis and χ2 tests for competence area analysis. Results: Ethnic minority GP trainees were more likely to face underperformance assessments than the majority group (odds ratio [OR] 2.41, 95% confidence interval [CI] = 1.67 to 3.49). Underperformance was not significantly nested in particular competence areas. First-generation ethnic minority trainees seemed more at risk than their second-generation peers. Conclusion: Ethnic minority GP trainees seem more at risk of facing educational barriers than the majority group. Additional qualitative research on underlying factors is essential.

Research in social work on coping and migration

This article focuses on religious and non religious coping strategies of migrants of the first, second and third generation Muslims of rural Moroccan background in the Netherlands. In this study it was found that in dealing with stressful events and difficult emotions, the younger generations make still use of the religious coping strategies of the first generation but combine them with more active problem-solving behaviour and a less deferring, more collaborative religious coping style towards God.

Guidance by physicians and pharmacists during antidepressant therapy

Background: Guidance of patients treated with antidepressants is paramount for successful therapy. The aim was to assess patients’ needs and suggestions for improvement of guidance by physicians and pharmacists during second generation antidepressant (SGA) therapy. Methods: Five focus group discussions were held with a total of 34 patients using an SGA. The discussions were conducted flexibly and responsively using a semi-structured topic list. All focus group discussions were videorecorded and transcripts were analyzed using ATLAS.ti for coding, thematic and open analysis. Results: Participants stated they were in need of better guidance. They suggested improving content of information during decisional moments, patient-health care professional communication and communication between health care professionals, and finally, organization of guidance. Barriers to achieving improved guidance were cited. Conclusions: Content, communication and organization of guidance are pivotal for achieving optimal guidance. Participants mentioned their current experienced guidance had limitations and brought up solutions for improvement. A next step would be to discuss the suggested solutions with health care professionals to assess their views and to discuss the possibility for implementation. After implementation, future studies could be aimed at determination of its impact on patients’ treatment efficacy, quality of life, treatment satisfaction and healthcare costs.
