Free newspapers may offer different news contents to different audiences, when compared with traditional, paid-for newspapers, but they, nevertheless, concentrate on news, and thereby provide society with information on current affairs. These papers have seen circulation rise until 2008; after that, a decline set in, leading to closures and often a monopoly situation in the mature European newspaper markets covered in our research. Free newspapers seem to follow a typical life cycle pattern, moving from growth to maturity, and to saturation and decline. Diversification strategies – home-delivery, weekend, sports, afternoon, and financial – have been disappointing so far. There is no evidence, however, of total extinction, indicating that there is room for at least one title – possibly two – in every market. The situation in the surveyed markets also suggests that a free newspaper may be a ‘natural’ monopoly.

Whereas most continents see circulation of newspapers drop, Latin America is one of the few areas in the world with rising newspaper circulation. A substantial part of this rise is caused by the introduction of free newspapers in a dozen countries on the continent. We map this development, discuss possible reasons for the rapid growth and try to answer whether this introduction has substitution effects on paid newspapers. The growth of this new format is most probably caused by economic growth while we conclude that substitution is low, meaning that free dailies serve a new reading audience in Latin America

Background & aims: In dietary practice, it is common to estimate protein requirements on actual bodyweight, but corrected bodyweight (in cases with BMI <20 kg/m2 and BMI ≥30 kg/m2) and fat free mass (FFM) are also used. Large differences on individual level are noticed in protein requirements using these different approaches. To continue this discussion, the answer is sought in a large population to the following question: Will choosing actual bodyweight, corrected bodyweight or FFM to calculate protein requirements result in clinically relevant differences? Methods: This retrospective database study, used data from healthy persons ≥55 years of age and in- and outpatients ≥18 years of age. FFM was measured by air displacement plethysmography technology or bioelectrical impedance analysis. Protein requirements were calculated as 1) 1.2 g (g) per kilogram (kg) actual bodyweight or 2) corrected bodyweight or 3) 1.5 g per kg FFM. To compare these three approaches, the approach in which protein requirement is based on FFM, was used as reference method. Bland–Altman plots with limits of agreement were used to determine differences, analyses were performed for both populations separately and stratified by BMI category and gender. Results: In total 2291 subjects were included. In the population with relatively healthy persons (n = 506, ≥55 years of age) mean weight is 86.5 ± 18.2 kg, FFM is 51 ± 12 kg and in the population with adult in- and outpatients (n = 1785, ≥18 years of age) mean weight is 72.5 ± 18.4 kg, FFM is 51 ± 11 kg. Clinically relevant differences were found in protein requirement between actual bodyweight and FFM in most of the participants with overweight, obesity or severe obesity (78–100%). Using corrected bodyweight, an overestimation in 48–92% of the participants with underweight, healthy weight and overweight is found. Only in the Amsterdam UMC population, protein requirement is underestimated when using the approach of corrected bodyweight in participants with severe obesity. Conclusion: The three approaches in estimation of protein requirement show large differences. In the majority of the population protein requirement based on FFM is lower compared to actual or corrected bodyweight. Correction of bodyweight reduces the differences, but remain unacceptably large. It is yet unknown which method is the best for estimation of protein requirement. Since differences vary by gender due to differences in body composition, it seems more accurate to estimate protein requirement based on FFM. Therefore, we would like to advocate for more frequent measurement of FFM to determine protein requirements, especially when a deviating body composition is to be expected, for instance in elderly and persons with overweight, obesity or severe obesity.

Although a third of papers closed down in the last ten years, newspaper circulation in most countries with free newspapers is actually higher than before their entry. Readership analysis of free newspapers shows that these papers have a younger audience than paid newspapers, contradicting the notion that young people don’t read newspapers anymore.

The EU is confident it will reach its target of 20% renewable energy by 2020. But according to Martien Visser, professor at the Hanze University of Applied Sciences in Groningen (The Netherlands), this 20% is in reality more like 14%. This is because a large part of our energy consumption is simply ignored in the calculations for renewable energy. “Even with 100% renewables, we would still need a lot of fossil fuels”, Visser notes.

Objectives Patients who underwent corrective surgery for tetralogy of Fallot (TOF) have increased long-term risk of cardiovascular morbidity and mortality. Yet, limited information is available on how to evaluate the risk in this population. Therefore, the aim of this study was to investigate the prognostic value of aerobic exercise capacity, along with other related parameters, at medium-term follow-up in adult patients with tetralogy of Fallot. Methods and results Between 2000 and 2003, 92 adults (age 26.2 ± 7.8 years; 63 male) with corrected TOF or TOF-type morphology underwent a cardiopulmonary exercise test (CPET) until exhaustion and echocardiography. During a mean follow-up of 7.3 ± 1.2 years (range 0.9 to 9.3 years), 2 patients died and 26 patients required at least 1 cardiac-related intervention at a mean age of 28.9 ± 7.9 years. Event-free survival tended to be higher in patients with the classical type of TOF (P = 0.061). At multivariate Cox analysis, age at CPET [hazard ratio (HR): 1.13, P = 0.006], age at correction (HR: 0.82, P = 0.037), right ventricular (RV) function (HR: 4.94, P = 0.001), QRS duration (HR: 1.02, P = 0.007), percentage of predicted peak oxygen uptake (peak VO2%) (HR: 0.96, P = 0.029) and ventilatory effi ciency slope (VE/VCO2 slope) (HR: 1.13, P = 0.021) were signifi cantly related to the incidence of death/cardiac-related intervention during medium follow-up. Conclusions Early corrective surgery and a well-preserved RV are associated with a better outcome in adults with corrected TOF. Furthermore, CPET provides important prognostic information; peak VO2% and VE/VCO2 slope are independent predictors for event-free survival in patients with corrected TOF.

In 1999 werden de gratis kranten Metro en Spits in Nederland gelanceerd. Behalve die twee titels werden ook zes andere gratis dagbladen in Nederland gestart tussen 2000 en 2008. In 2007 bereikte de totale gratis oplage een hoogtepunt van bijna twee miljoen per dag. In dit artikel wordt de geschiedenis van de deze categorie kranten beschreven tegen de Europese achtergrond waar ook sprake is van een ‘levenscyclus’ met een snelle opkomst en een sterke sanering die tijdens de economische recessie inzette. Daarnaast wordt ingegaan op de voorlopers van deze mediasoort, op de lezers van gratis kranten en de gevolgen die gratis kranten op de totale krantenoplage in Nederland heeft gehad. Halfway the first decennium of the 21st century free newspapers in the Netherlands had a circulation of almost two million and were responsible for a third of the total newspaper circulation. Four national titles competed. They saw a quick rise and also a sudden decline after 2008. In 2015 only one national model remains. In this article we analyse this development against the European background as several countries in Europe show the same picture. Also the historical context of the global rise of free newspapers – going back to the late 19th century – and kickstarted in 1995 by Metro Sweden -is provided. As free newspapers rely only on advertising for income, a combination of economic recessions and fierce competition can be lethal. Free newspapers mainly compete among themselves which explained the sudden decline in the number of titles and circulation after 2008. Also the rise of digital media could be harmful for free newspapers as they provide a attractive alternative for young readers, the target group for free newspapers. There always has been a high overlap between the readership of free and paid newspapers, suggesting low substitution. Free newspapers seem to follow a pattern of a classic ‘life cycle’ model.

One of the factors that differentiate honors from regular teaching at the Faculty of Geosciences at Utrecht University, the Netherlands, is the freedom that honors students enjoy, a freedom that evokes excellence because it is focused and targeted. This targeted freedom takes three different shapes in our honors program and comes with specific challenges for both students and teachers. While the attractions and advantages of such freedom are both theoretically and practically significant, our experience has also demonstrated drawbacks that need to be addressed and resolved in creating effective honors education.The challenges and struggles as well as the rewards that we have experienced might be familiar to honors educators around the world, but they are also shaped by the particular contexts of our program within the Faculty of Geosciences, within Utrecht University, and within the Netherlands, contexts that we will now introduce.
