Jaarlijks worden ongeveer 80.000 patiënten behandeld op de ruim 80 intensive care-afdelingen in Nederland.Op een intensive careafdeling worden vitale functies bewaakt en meestal zelfs overgenomen. Bij de meeste patiënten is voor kortere of langere tijd kunstmatige beademing noodzakelijk. Kunstmatige beademing is effectief en soms zelfs levens reddend maar is in het geheel niet zonder risico’s. Lector Critical Care Frederique Paulus gaat in haar rede in op de uitdagingen die de interprofessionele teams op de Intensive Care hebben ten aanzien van de luchtweg- en beademingszorg. Zij zal proberen te schetsen wat ‘Wij gaan goed voor u zorgen’ op een Intensive Care betekent. Het bijzonder lectoraat CriticalCare is ingesteld in samenwerking methet Amsterdam UMC locatie AMC.

OBJECTIVE: To describe the long-term functioning of patients who survived a COVID-19-related admission to the intensive care unit and their family members, in the physical, social, mental and spiritual domain.DESIGN: A single-centre, prospective cohort study with a mixed-methods design.SETTING: The intensive care unit of the University Medical Center Groningen in the Netherlands.MAIN OUTCOME MEASURES: To study functioning 12 months after intensive care discharge several measurements were used, including a standardised list of physical problems, the Clinical Frailty Scale, the Medical Outcomes Study Short-Form General Health Survey, the McMaster Family Assessment Device, the Hospital Anxiety and Depression Scale, and the Spiritual Needs Questionnaire, as well as open questions and interviews with survivors and their family members.RESULTS: A total of 56 survivors (77%) returned the 12-month questionnaire, whose median age was 62 (inter-quartile range [IQR]: 55.0-68.0). Moreover, 67 family members (66%) returned the 12-month questionnaire, whose median age was 58 (IQR: 43-66). At least one physical problem was reported by 93% of the survivors, with 22% reporting changes in their work-status. Both survivors (84%) and their family members (85%) reported at least one spiritual need. The need to feel connected with family was the strongest. The main theme was 'returning to normal' in the interviews with survivors and 'if the patient is well, I am well' in the interviews with family members.CONCLUSIONS: One year after discharge, both COVID-19 intensive care survivors and their family members positively evaluate their health-status. Survivors experience physical impairments, and their family members' well-being is strongly impacted by the health of the survivor.

Purpose: Lactate is an established prognosticator in critical care. However, there still is insufficient evidence about its role in predicting outcome in COVID-19. This is of particular concern in older patients who have been mostly affected during the initial surge in 2020. Methods: This prospective international observation study (The COVIP study) recruited patients aged 70 years or older (ClinicalTrials.gov ID: NCT04321265) admitted to an intensive care unit (ICU) with COVID-19 disease from March 2020 to February 2021. In addition to serial lactate values (arterial blood gas analysis), we recorded several parameters, including SOFA score, ICU procedures, limitation of care, ICU- and 3-month mortality. A lactate concentration ≥ 2.0 mmol/L on the day of ICU admission (baseline) was defined as abnormal. The primary outcome was ICU-mortality. The secondary outcomes 30-day and 3-month mortality. Results: In total, data from 2860 patients were analyzed. In most patients (68%), serum lactate was lower than 2 mmol/L. Elevated baseline serum lactate was associated with significantly higher ICU- and 3-month mortality (53% vs. 43%, and 71% vs. 57%, respectively, p < 0.001). In the multivariable analysis, the maximum lactate concentration on day 1 was independently associated with ICU mortality (aOR 1.06 95% CI 1.02–1.11; p = 0.007), 30-day mortality (aOR 1.07 95% CI 1.02–1.13; p = 0.005) and 3-month mortality (aOR 1.15 95% CI 1.08–1.24; p < 0.001) after adjustment for age, gender, SOFA score, and frailty. In 826 patients with baseline lactate ≥ 2 mmol/L sufficient data to calculate the difference between maximal levels on days 1 and 2 (∆ serum lactate) were available. A decreasing lactate concentration over time was inversely associated with ICU mortality after multivariate adjustment for SOFA score, age, Clinical Frailty Scale, and gender (aOR 0.60 95% CI 0.42–0.85; p = 0.004). Conclusion: In critically ill old intensive care patients suffering from COVID-19, lactate and its kinetics are valuable tools for outcome prediction. Trial registration number: NCT04321265.

PURPOSE: Clinical examination is often the first step to diagnose shock and estimate cardiac index. In the Simple Intensive Care Studies-I, we assessed the association and diagnostic performance of clinical signs for estimation of cardiac index in critically ill patients.METHODS: In this prospective, single-centre cohort study, we included all acutely ill patients admitted to the ICU and expected to stay > 24 h. We conducted a protocolised clinical examination of 19 clinical signs followed by critical care ultrasonography for cardiac index measurement. Clinical signs were associated with cardiac index and a low cardiac index (< 2.2 L min-1 m2) in multivariable analyses. Diagnostic test accuracies were also assessed.RESULTS: We included 1075 patients, of whom 783 (73%) had a validated cardiac index measurement. In multivariable regression, respiratory rate, heart rate and rhythm, systolic and diastolic blood pressure, central-to-peripheral temperature difference, and capillary refill time were statistically independently associated with cardiac index, with an overall R2 of 0.30 (98.5% CI 0.25-0.35). A low cardiac index was observed in 280 (36%) patients. Sensitivities and positive and negative predictive values were below 90% for all signs. Specificities above 90% were observed only for 110/280 patients, who had atrial fibrillation, systolic blood pressures < 90 mmHg, altered consciousness, capillary refill times > 4.5 s, or skin mottling over the knee.CONCLUSIONS: Seven out of 19 clinical examination findings were independently associated with cardiac index. For estimation of cardiac index, clinical examination was found to be insufficient in multivariable analyses and in diagnostic accuracy tests. Additional measurements such as critical care ultrasonography remain necessary.

The aim of this study was to develop and validate an algorithm that can identify the type, frequency, and duration of activities common to intensive care (IC) patients. Ten healthy participants wore two accelerometers on their chest and leg while performing 14 activities clustered into four protocols (i.e., natural, strict, healthcare provider, and bed cycling). A video served as the reference standard, with two raters classifying the type and duration of all activities. This classification was reliable as intraclass correlations were all above 0.76 except for walking in the healthcare provider protocol, (0.29). The data of four participants were used to develop and optimize the algorithm by adjusting body-segment angles and rest-activity-threshold values based on percentage agreement (%Agr) with the reference. The validity of the algorithm was subsequently assessed using the data from the remaining six participants. %Agr of the algorithm versus the reference standard regarding lying, sitting activities, and transitions was 95%, 74%, and 80%, respectively, for all protocols except transitions with the help of a healthcare provider, which was 14-18%. For bed cycling, %Agr was 57-76%. This study demonstrated that the developed algorithm is suitable for identifying and quantifying activities common for intensive care patients. Knowledge on the (in)activity of these patients and their impact will optimize mobilization.

Background: The COVID-19 pandemic has led highly developed healthcare systems to the brink of collapse due to the large numbers of patients being admitted into hospitals. One of the potential prognostic indicators in patients with COVID-19 is frailty. The degree of frailty could be used to assist both the triage into intensive care, and decisions regarding treatment limitations. Our study sought to determine the interaction of frailty and age in elderly COVID-19 ICU patients. Methods: A prospective multicentre study of COVID-19 patients ≥ 70 years admitted to intensive care in 138 ICUs from 28 countries was conducted. The primary endpoint was 30-day mortality. Frailty was assessed using the clinical frailty scale. Additionally, comorbidities, management strategies and treatment limitations were recorded. Results: The study included 1346 patients (28% female) with a median age of 75 years (IQR 72–78, range 70–96), 16.3% were older than 80 years, and 21% of the patients were frail. The overall survival at 30 days was 59% (95% CI 56–62), with 66% (63–69) in fit, 53% (47–61) in vulnerable and 41% (35–47) in frail patients (p < 0.001). In frail patients, there was no difference in 30-day survival between different age categories. Frailty was linked to an increased use of treatment limitations and less use of mechanical ventilation. In a model controlling for age, disease severity, sex, treatment limitations and comorbidities, frailty was independently associated with lower survival. Conclusion: Frailty provides relevant prognostic information in elderly COVID-19 patients in addition to age and comorbidities. The objective of this study is to illuminate the effects of intensive aid on the self-reliance and independence of single mothers, and to show what is and what can be done to improve the empowerment of this group. Firstly the paper focuses on the effects of intensive social aid on the self-reliance and independence of single mothers, and how this affects the process of regaining social inclusion in each of the home countries. Secondly the results are compared with the second country, in respect of the socio-economic and cultural circumstances. This study is based upon a comparative data collected from randomly selected single mothers in each country. In the Netherlands information was collected during in-depth interviews; in Cyprus semi-structured interviews and questionnaires were used. The aim is to shed light on the issues that impact on the lives and the well-being of these mothers and their children in their social context. This chapter presents sections of two large research projects which attempted to investigate the single-mother families from the woman’s perspective, and it aims to analyse the following: • Factors that hinder and improve the independent life of a single mother • Choices that women make during the offered aid period and their motives • The results of this change.

Dit hoofdstuk behandelt tal van onderwerpen uit de dagelijkse praktijk op de intensive care. Allereerst belichten we het assessment en de eerste opvang van de vitaal bedreigde patiënt volgens de ABCDE-systematiek. Vervolgens de aandachtspunten en wettelijke eisen die gesteld worden aan rapportage en verpleegkundige verslaglegging. We belichten scoringssystemen om zaken als de ernst van aandoeningen, sterftekans, diepte van sedatie, pijn, werklast en overlevingskans te kunnen classificeren: APACHE, SOFA, SAPS, CAM ICU en RASS, (TISS), frailty-index, MEWS en SF-36/RAND-36. Na een uiteenzetting over debriefing is er aandacht voor lichamelijke en persoonlijke verzorging en mobilisatie van de IC-patiënt, en voor de gevolgen van immobiliteit. Comorbiditeit en multimorbiditeit blijven evenmin onbelicht, evenals thermoregulatie met aandacht voor hyperthermie en hypothermie. Het fenomeen pijn, en daaraan gekoppeld pijnbestrijding/analgesie, wordt besproken. Ook slaapproblemen, Family-Centered Care (PFCC), ofwel familiegerichte zorg, en crisismanagement worden besproken. Ten slotte is er aandacht voor klinische ethiek. Hierbij spelen onder andere hersendood en orgaandonatie een belangrijke rol.
