Voor het meten van de mobiliteit bij geriatrische patiënten en bij patiënten met evenwichtsstoornissen is in 1986 door Mathias de Timed Up and Go test (TUG) ontwikkeld. Deze oorspronkelijk Engelstalige versie van de test werd in 2000 in het Nederlands vertaald door De Jong. Het meetinstrument is zowel evaluatief als inventariserend en wordt aanbevolen in de KNGF-richtlijnen ‘Osteoporose’, ‘Beroerte’ en ‘Ziekte van Parkinson’.

Dysarthritic Parkinson speech is characterised by impairment of expressive linguistic prosody, even making it difficult to understand. While rigidity and bradykinesia can be held responsible for a general decline in speaking ability, the origin of prosodic impairment must be seen in the light of the accompanying impairments of receptive prosody such as the inability to recognize intonational meaning and make lexical distinctions based on stress contrasts . The stimulating effect of music on motor coordination in afflicted patients suggests that music might have a similar effect on vocal behavior. It could be hypothesized that the singing of Parkinson patients might remain relatively unaffected by the disease. In this study, vocal improvisation was used to compare the singing of Parkinson patients with that of healthy controls, matched for age and gender. When F0 , range, mean absolute slope, and tempo were contrasted, Parkinson patients did not differ significantly from controls.

Parkinson’s disease is characterized not only by bradykinesia, rigidity, and tremor, but also by impairments of expressive and receptive linguistic prosody. The facilitating effect of music with a salient beat on patients’ gait suggests that it might have a similar effect on vocal behavior, however it is currently unknown whether singing is affected by the disease. In the present study, fifteen Parkinson patients were compared with fifteen healthy controls during the singing of familiar melodies and improvised melodic continuations. While patients’ speech could reliably be distinguished from that of healthy controls matched for age and gender, purely on the basis of aural perception, no significant differences in singing were observed, either in pitch, pitch range, pitch variability, and tempo, or in scale tone distribution, interval size or interval variability. The apparent dissociation of speech and singing in Parkinson’s disease suggests that music could be used to facilitate expressive linguistic prosody.

Het lectoraat Lifelong Learning in Music wil een bijdrage leveren aan de ontwikkeling van musici tot lerende, onderzoekende en ondernemende professionals die midden in de samenleving staan. Het lectoraat doet dat door de verschillende rollen die musici kunnen vervullen en de ontwikkeling van hun leiderschap te onderzoeken in relatie tot hun levenslange persoonlijke, artistieke en professionele ontwikkeling. Centraal staat daarbij de vraag wat het betekent voor musici om, op grond van een fundamenteel begrip van de verschillende culturele en sociale contexten waarbinnen zij werken, innovatieve praktijken met nieuw publiek vorm te geven. Het lectoraat Lifelong Learning in Music is verbonden aan het Prins Claus Conservatorium en onderdeel van het kenniscentrum Kunst en Samenleving van de Hanzehogeschool Groningen. Lifelong learning kan kort omschreven worden als het vermogen (van musici) om adaptief en proactief te reageren op veranderingen in de samenleving, ‘transforming experience into knowledge, skills, attitudes, values, emotions, beliefs and the senses’ (Jarvis 2002, 60). Het doel om innovatieve praktijken te ontwikkelen vindt zijn beslag in een grote onderzoekslijn van het lectoraat, Healthy Ageing through Music & the Arts. The realization of one’s musical ideas at the keyboard is dependent on the ability to transform sound into movement, a process called audiomotor transformation. Using fMRI, we investigated cerebral activations while classically-trained improvising and non-improvising musicians imagined playing along with recordings of familiar and unfamiliar music excerpts. We hypothesized that audiomotor transformation would be associated with the recruitment of dedicated cerebral networks, facilitating aurally-cued performance. Results indicate that while all classically-trained musicians engage a left-hemisphere network involved in motor skill and action recognition, only improvising musicians additionally recruit a right dorsal frontoparietal network dedicated to spatially-driven motor control. Mobilization of this network, which plays a crucial role in the real-time transformation of imagined or perceived music into goal-directed action, may be held responsible not only for the stronger activation of auditory cortex we observed in improvising musicians in response to the aural perception of music, but also for the superior ability to play ‘by ear’ which they demonstrated in a follow-up study. The results of this study suggest that the practice of improvisation promotes the implicit acquisition of hierarchical music syntax which is then recruited in top-down manner via the dorsal stream during music performance. In a study of audiomotor transformation in Parkinson patients, we demonstrated a dissociation between dysprosody in speech and music. While patients’ speech could reliably be distinguished from that of healthy individuals, purely on the basis of aural perception, no difference was observed between patients and healthy controls in their ability to sing improvised melodies.

BACKGROUND: Integrated care is essential for improving the management and health outcomes for people with Parkinson's disease (PD); reliable and objective measures of care integration are few.OBJECTIVE: The aim of this study was to test the psychometric properties of the Rainbow Model of Integrated Care Measurement Tool (RMIC-MT, provider version) for healthcare professionals involved in PD care.METHODS: A cross-sectional survey was administered online to an international network representing 95 neurology centers across 41 countries and 588 healthcare providers. Exploratory factor analysis with principal axis extraction method was used to assess construct validity. Confirmatory factor analysis was used to evaluate model fit of the RMIC-MT provider version. Cronbach's alpha was used to assess the internal consistency reliability.RESULTS: Overall, 371 care providers (62% response rate) participated in this study. No item had psychometric sensitivity problems. Nine factors (professional coordination, cultural competence, triple aims outcome, system coordination, clinical coordination, technical competence, community-centeredness, person-centeredness, and organizational coordination) with 42 items were determined by exploratory factor analysis. Cronbach's alpha ranged from 0.76 (clinical coordination) to 0.94 (system coordination) and showed significant correlation among all items in the scale (>0.4), indicating good internal consistency reliability. The confirmatory factor analysis model passed most goodness-of-fit tests, thereby confirming the factor structure of nine categories with a total of 40 items.CONCLUSIONS: The results provide evidence for the construct validity and other psychometric properties of the provider version of the RMIC-MT to measure integrated care in PD. © 2023 The Authors. Onder NAH verstaan we de mensen die ergens in hun leven een beschadiging aan hun hersenen hebben opgelopen zoals door een hersentumor, een ongeluk, een beroerte, een zuurstoftekort of door een vergiftiging. Ook progressieve ziekten zoals Dementie of Parkinson worden onder hersenletsel gerekend. Er zijn in Nederland 500.000 mensen die met hersenletsel leven.

BACKGROUND: Although physical activity is beneficial for Parkinson's disease (PD) patients, many do not meet the recommended levels. The range of physical activity among sedentary PD patients is unknown, as are factors that determine this variability. Hence, we aimed to (1) assess daily physical activity in self-identified sedentary PD patients; (2) compare this with criteria of a daily physical activity guideline; and (3) identify determinants of daily physical activity. METHODS: Daily physical activity of 586 self-identified sedentary PD patients was measured with a tri-axial accelerometer for seven consecutive days. Physical fitness and demographic, disease-specific, and psychological characteristics were assessed. Daily physical activity was compared with the 30-min activity guideline. A linear mixed-effects model was estimated to identify determinants of daily physical activity. RESULTS: Accelerometer data of 467 patients who fulfilled all criteria revealed that >98% of their day was spent on sedentary to light-intensity activities. Eighty-two percent of the participants were 'physically inactive' (0 days/week of 30-min activity); 17% were 'semi-active' (1-4 days/week of 30-min activity). Age, gender, physical fitness, and scores on the Unified Parkinson's Disease Rating Scale explained 69% of the variability in daily physical activity. CONCLUSIONS: Performance-based measurements confirmed that most self-identified sedentary PD patients are 'physically inactive'. However, the variance in daily physical activity across subjects was considerable. Higher age, being female, and lower physical capacity were the most important determinants of reduced daily physical activity. Future therapeutic interventions should aim to improve daily physical activity in these high-risk patients, focusing specifically on modifiable risk factors.

De fysische, chemische en microbiologische gevaren van het opwerken van vezelcomponenten uit reststromen van uien zijn geanalyseerd op basis van literatuuronderzoek. Uienreststromen zijn geschikt voor het winnen van olie door middel van stoomdestillatie of eiwitten door middel van iso-elektrische precipitatie. Bij deze processen wordt ook de uienschil verwerkt. Er blijft o.a. een vezelrijke fractie over die in principe geschikt is voor humane consumptie. Fysische vreemde delen vormen zeer zelden een acuut risico voor de gezondheid. De meest voorkomende pesticiden op ui zijn maleïnehydrazide, fluopyram en fipronil. Incidenteel kan de maximaal toelaatbare hoeveelheid van een pesticide overschreden worden, maar dit heeft geen acute nadelige gezondheidsgevolgen. Van zware metalen is er alleen Europese wetgeving voor gehaltes aan lood en cadmium in ui. Microbiologische gevaren voor de processen zijn gerelateerd aan vegetatieve cellen, toxines of sporen van pathogenen. Vegetatieve cellen zijn alleen een risico voor onverhitte vezelfracties of na kruisbesmetting. Toxines kunnen nog actief zijn na stoomdestillatie en ook na pasteurisatie van eiwitpasta. Hetzelfde geldt voor de sporen van bacteriën. Om ontkieming van sporen te voorkomen moet de uienstroom boven 48 °C gehouden worden of snel worden gekoeld .
