Background and Purpose: Despite the positive effects of physical activity on numerous aspects of health, many older adults remain sedentary even after participating in physical activity interventions. Standardized exercise programs do not necessarily bring about the behavioral change that is necessary. Therefore, a patient-centered approach is needed. The purpose of this study was to develop and assess the acceptabilityand potential effectiveness of the Coach2Move strategy; a physical therapy (PT) approach aimed at improving the longterm level of physical activity in mobility-limited older adults. Methods: The Coach2Move strategy was developed on the basis of 2 systematic literature studies and expert consultations. Multiple focus group meetings and a Delphi procedure were organized to gain consensus on the Coach2Move strategy.Acceptability and potential effectiveness were studied in a pilot study with a pre-/postdesign in which 2 physical therapists and 12 patients participated. To assess acceptability, patients were interviewed, discussion with the involved physical therapists was held, and health records were studied.Potential effectiveness was tested measuring the level of physical activity, frailty, quality of life, and mobility before and after treatment. Results: The pilot study showed high appraisal of the strategy by both physical therapists and patients. Moreover, a potential effect on the level of physical activity, frailty, quality of life, and mobility was observed. In conclusion, the Coach2Move strategy can be considered acceptable in PT practice and showed potential benefits.

BACKGROUND In contrast to several other countries, smoking is not an integral part of treatment during admission to a psychiatric hospital in The Netherlands. AIM Implementation of a smoking cessation program for patients and employees of a psychiatric ward of an academic medical center in The Netherlands. METHOD Prospective, mixed-method study of implementation of a smoking cessation program for patients and employees of a psychiatric academic hospital in Amsterdam. The program consisted of 7 weekly group meetings by certified smoking cessation coaches. Nicotine replacement therapy was provided for free, if necessary. RESULTS During 14 months, 65 individuals were seeking help to stop smoking: 39 patients and 26 employees. Of these, 29 patients and 16 employees participated in group meetings with an average of 2.6 times per person. There were 20 individuals who visited the group meetings or received individual coaching at least 3 times (6 patients and 14 employees). Fifty-five percent of these individuals reported to be smoke-free at 3 months after joining the first meeting. Employees were much more likely to quit than patients. From interviews with 20 participants, it was noticed that combining patients and employees in one group was perceived as a barrier due to a gap in processing speed. CONCLUSION On the psychiatric ward of an academic hospital in The Netherlands, there was a positive experience with providing smoking cessation treatment. A small number of employees and patients participated in a smoking cessation program and quitting smoking was reached by only a few patients. Supporting smoking cessation in a psychiatric hospital asks for intensive screening, diagnosing, treatment and smoke-free policies.

Tot in de jaren negentig van de vorige eeuw is er sprake van een gebrek aan aandacht voor de somatische problemen van psychiatrische en verslaafde patiënten in instellingen voor ggz en maatschappelijke opvang. Ook ontbreekt het aan een goede ketensamenwerking met betrekking tot deze problematiek. Binnen een samenwerkingsverband van de gemeente Utrecht, zorginstellingen en huisartsen is deze gebrekkige aandacht in de afgelopen tien jaar een belangrijk thema geweest, uiteindelijk (in 2012) resulterend in de Utrechtse Somatische Screeningslijst. Met deze screening wordt beoogd somatische gezondheidsproblemen vroegtijdig te signaleren. Daarmee kan een beter beeld gevormd worden van de gezondheid van de patiënten en kan de huisarts eerder worden ingeschakeld. De eerste afgenomen screenings laten zien dat veelvoorkomende klachten en aandoeningen bij deze patiënten zijn: ondergewicht, COPD- en andere longklachten, pijnklachten, epileptische aanvallen en huidaandoeningen. Veel patiënten dragen een gebitsprothese. De verpleegkundigen ervaren de screening als een positieve ontwikkeling binnen hun vakgebied. Er zijn echter ook knelpunten aan te merken. Besloten is de USS bij te stellen, op basis van de eerste bevindingen en van een enquête onder degenen die ervaring met het instrument hebben opgedaan. Met de aangepaste lijst wordt een onderzoek uitgevoerd binnen Victas.
