The so called Second Demographic Transition (Lesthaeghe and Van der Kaa, 1986), which surfaced in the sixties of the twentieth century in Western Europe and North America, resulted from a significant change in the pattern of norms and values. This again resulted in delayed fertility, a declining population when there was no replenishment through "replacement migration” and an increasing variety of household structures (with a rising number of one-person households). The rise in life expectancy coupled with a declining fertility, evolved into a gradual ageing of the population.The concept of ‘unbalanced population decline’ (Van Nimwegen and Heering 2009) enables us, while studying population decline, to take into account different motives underlying the decision to migrate during the life course; young people migrating in search of higher education and job opportunities and elderly clustering in places with a high facility level. This unbalanced population decline is taking place in some rural parts and smaller towns in the Netherlands. Especially the two migration flows mentioned above determine the structure of the population and the possibilities for effective family, kin and other social support systems for the elderly.Method:Analysis using amongst others recent demographic data from de community of Oldambt (Netherlands)Results:It will be shown that the effects of the demographic transition in the North of the Netherlands are accelerated because of unbalanced population decline. Furthermore it will be argued that because of this combination in parts of the Northern Netherlands family, kin and social support systems for the elderly are deteriorating more rapidly than in other parts of the county.Conclusion:Combined effects of demographic transition and unbalanced population decline urge for a reconsideration of the possibilities to rely on family, kin and other social support systems in different regions in the Netherlands.

Background: The increase in life expectancy has brought about a higher prevalence of chronic illnesses among older people. Objectives: To identify common chronic illnesses among older adults, to examine the influence of such conditions on their Health-Related Quality of Life (HRQoL), and to determine factors predicting their HRQoL. Method: A population-based cross-sectional study was conducted involving 377 individuals aged 60 years and above who were selected using multi-stage sampling techniques in Olorunda Local Government, Osun State, Nigeria. Data were collected using an interviewer-administered questionnaire comprising socio-demographic characteristics, chronic illnesses, and the World Health Organization quality of life instrument (WHOQOL-BREF) containing physical health, psychological, social relationships, and environmental domains. Results: About half (51.5%) of the respondents reported at least one chronic illness which has lasted for 1–5 years (43.3%). The prevalence of hypertension was 36.1%, diabetes 13.9% and arthritis 13.4%. Respondents with chronic illness had significantly lower HRQoL overall and in the physical health, social relationships and the environmental domains (all p<0.05) compared to those without a chronic illness. Factors that predicted HRQoL include age, marital status, level of education, the presence of chronic illness and prognosis of the condition. Conclusion: This study concluded that chronic illness is prevalent in Nigerian older people and significantly influence their HRQoL. Age, marital status, and level of education were associated with HRQoL in this group.

The structure and financing of collective long-term care and support in the Netherlands changed dramatically with the introduction of the Social Support Act (WMO) the 1st of January 2015. This act arranged that municipalities assist people to live in their homes for as long as possible by providing various forms of aid and support. This aid and support however, is in addition to the help that people arrange for themselves and / or receive in the form of volunteer aid (mantelzorg) by family, friends and neighbours. Furthermore under this Act people only become eligible for state financed sheltered accommodation in the exceptional case of severe physical or mental illness. The fact that under the new regime of the Social Support Act municipal support is made additional to volunteer aid, it is important to understand what role family, friends and neighbours potentially can play and in fact do play. This is especially important in regions were unbalanced population decline through out-migration of young people, effectively changes social structures and accelerates the ageing of the population. For this reason we designed a limited pilot-study to test the possibilities and the necessity of a broader study focussing on potential and every day practice concerning volunteer aid for elderly in regions characterized by unbalanced population decline. In this pilot study we will focus on a rural municipality in the north of the Netherlands: Oldambt.Within the framework of this pilot-study we focussed on living arrangements and social network of two groups of senior citizens; one group of men and women aged 65-79 and one group aged 80 years and over. Based on demographic data kindly made available by the municipal office of Oldambt, in this paper we will draw a picture of these citizens living in the municipality. Going deeper into the material and the municipality’s structure we will than focus on one of the municipality’s communities, Finsterwolde, with its village, hamlets and surrounding rural area. For the purpose of this pilot-study we constructed a sample of 30 men and women aged 80+-elderly and asked them to fill in a questionnaire. Based on the outcomes of these questionnaires we than had five in-depth interviews with some of them. Finally, on the basis of the gathered material, we will draw some general conclusions while presenting some new questions for further research into the living conditions and social support systems for elderly in a region with unbalanced population decline and accelerated ageing.

This study focused on the influence of a transformational leadership style on professionals' willingness to change. This multiple case study was performed in three project management organisations that had recently implemented a new business information system. The research data were obtained through both qualitative and quantitative data collection. The qualitative investigation revealed that through leading by good example a manager has a positive influence on their employees' willingness to change. However, the quantitative investigation showed that there is no relationship between transformational leadership and the motivational factors of willingness to change. PURPOSE: The purpose of the study is to identify demographic, clinical, lifestyle-related, and social-cognitive correlates of physical activity (PA) intention and behavior in head and neck cancer (HNC) survivors using the theory of planned behavior (TPB).METHODS: Data from two cross-sectional studies on correlates of PA in HNC survivors were pooled. Both studies used self-reports to assess PA and social-cognitive correlates. Potential correlates were collected via self-report or medical records. Univariable and multivariable multilevel linear mixed-effects models were built to identify correlates of PA intention and PA behavior (Z scores). Structural equation model analyses were conducted to study the full TPB model in one analysis, taking into account relevant covariates.RESULTS: In total, 416 HNC survivors were surveyed. Their mean (SD) age was 66.6 (9.4) years; 64% were men, and 78% were diagnosed with laryngeal cancer. The structural equation model showed that PA intention was significantly higher in HNC survivors with a history of exercising, who had a more positive attitude, subjective norm, and perceived behavioral control. Patients with higher PA intention, higher PBC, a lower age, and without unintentional weight loss or comorbidities had higher PA behavior. The model explained 22.9% of the variance in PA intention and 16.1% of the variance in PA behavior.CONCLUSIONS: Despite significant pathways of the TPB model, the large proportion variance in PA intention and behavior remaining unexplained suggests the need for better PA behavior (change) models to guide the development of PA promotion programs, particularly for the elderly. Such programs should be tailored to comorbidities and nutritional status.

With the effects of climate change linked to the use of fossil fuels, as well as the prospect of their eventual depletion, becoming more noticeable, political establishment and society appear ready to switch towards using renewable energy. Solar power and wind power are considered to be the most significant source of global low-carbon energy supply. Wind energy continues to expand as it becomes cheaper and more technologically advanced. Yet, despite these expectations and developments, fossil fuels still comprise nine-tenths of the global commercial energy supply. In this article, the history, technology, and politics involved in the production and barriers to acceptance of wind energy will be explored. The central question is why, despite the problems associated with the use of fossil fuels, carbon dependency has not yet given way to the more ecologically benign forms of energy. Having briefly surveyed some literature on the role of political and corporate stakeholders, as well as theories relating to sociological and psychological factors responsible for the grassroots’ resistance (“not in my backyard” or NIMBYs) to renewable energy, the findings indicate that motivation for opposition to wind power varies. While the grassroots resistance is often fueled by the mistrust of the government, the governments’ reason for resisting renewable energy can be explained by their history of a close relationship with the industrial partners. Background: Medically unexplained physical symptoms (MUPS) are a leading cause of reduced work functioning. It is not known which factors are associated with reduced work functioning in people with moderate MUPS. Insight in these factors can contribute to prevention of reduced work functioning, associated work-related costs and in MUPS becoming chronic. Therefore, the aim of this study was to identify which demographic and health-related factors are associated with reduced work functioning, operationalized as impaired work performance and absenteeism, in people with moderate MUPS. Methods: Data of 104 participants from an ongoing study on people with moderate MUPS were used in this cross-sectional study. Ten independent variables were measured at baseline to determine their association with reduced work functioning: severity of psychosocial symptoms (four domains, measured with the Four-Dimensional Symptom Questionnaire), physical health (RAND 36-Item Health Survey), moderate or vigorous physical activity (Activ8 activity monitor), age, sex, education level and duration of complaints. Two separate multivariable linear regression analyses were performed with backward stepwise selection, for both impaired work performance and absenteeism. Results: Absenteeism rate rose with 2.5 and 0.6% for every increased point on the Four-Dimensional Symptom Questionnaire for domain 'depression' (B = 0.025, SE = 0.009, p = .006) and domain 'somatization' (B = 0.006, SE = 0.003, p = .086), respectively. An R2 value of 0.118 was found. Impaired work performance rate rose with 0.2 and 0.5% for every increased point on the Four-Dimensional Symptom Questionnaire for domain 'distress' (B = 0.002, SE = 0.001, p = .084) and domain 'somatization' (B = 0.005, SE = 0.001, p < .001), respectively. An R2 value of 0.252 was found. Conclusions: Severity of distress, probability of a depressive disorder and probability of somatization are positively associated with higher rates of reduced work functioning in people with moderate MUPS. To prevent long-term absenteeism and highly impaired work performance severity of psychosocial symptoms seem to play a significant role. However, because of the low percentage of explained variance, additional research is necessary to gain insight in other factors that might explain the variance in reduced work functioning even better.

Background: Previous studies found that 40-60% of the sarcoidosis patients suffer from small fiber neuropathy (SFN), substantially affecting quality of life. SFN is difficult to diagnose, as a gold standard is still lacking. The need for an easily administered screening instrument to identify sarcoidosis-associated SFN symptoms led to the development of the SFN Screening List (SFNSL). The usefulness of any questionnaire in clinical management and research trials depends on its interpretability. Obtaining a clinically relevant change score on a questionnaire requires that the smallest detectable change (SDC) and minimal important difference (MID) are known. Objectives: The aim of this study was to determine the SDC and MID for the SFNSL in patients with sarcoidosis. Methods: Patients with neurosarcoidosis and/or sarcoidosis-associated SFN symptoms (N=138) included in the online Dutch Neurosarcoidosis Registry participated in a prospective, longitudinal study. Anchor-based and distribution-based methods were used to estimate the MID and SDC, respectively. Results: The SFNSL was completed both at baseline and at 6-months’ follow-up by 89/138 patients. A marginal ROC curve (0.6) indicated cut-off values of 3.5 points, with 73% sensitivity and 49% specificity for change. The SDC was 11.8 points. Conclusions: The MID on the SFNSL is 3.5 points for a clinically relevant change over a 6-month period. The MID can be used in the follow-up and management of SFN-associated symptoms in patients with sarcoidosis, though with some caution as the SDC was found to be higher.
