Autoimmune antibody profiling plays a prominent role in both classification and prognosis of systemic sclerosis (SSc). In the last years novel autoantibodies have been discovered and have become available in diagnostic assays. However, standardization in autoimmune serology is lacking, which may have a negative impact on the added value of autoantibodies in diagnosis and prognosis of SSc. In this paper we describe the comparison of commercially available diagnostic assays for the detection of SSc-associated autoantibodies and explored the coexistence of multiple SSc-associated autoantibodies within patients.

This study explores the application of systemic design approaches used in a complex commercial context to create positive and sustainable change. The case study was a business case on sustainable parenthood, in which the company tried to balance its ambitions for environmental sustainability with the need to survive in a highly competitive market. In close collaboration with the internal business company stakeholders, a causal loop diagram was created. The diagram mapped relations between global relevant trends for emerging young adults within the DACH market, sustainability, and parenting as a business. Leverage points for systemic change were identified which were explored through in-depth user interviews (n=10). This process eventually identified ten systemic insights, translated into insight cards to facilitate business actions.Based on these combined approaches, the MINT framework (Mapping Interventions and Narratives for Transformation) was developed, with a strong emphasis on co-creation, iteration, translation, and communication of systemic interventions. However, while the internal business stakeholders and company representatives appreciated the bird’s eye view that systemic design gave them, they were challenged by the methods’ abstract language and translation of systemic insights into concrete action. To address this, the developed framework utilized systemic design artefacts such as a storytelling map and user-centred insight cards to facilitate a more comprehensible systemic design approach.Overall, this study provides a first attempt at creating an actionable systemic design framework that can be used in commercial settings to promote positive systemic change. Future research will require further validation.

Objectives: Pulmonary hypertension is one of the leading causes of death in systemic sclerosis. Early detection and treatment of pulmonary hypertension in systemic sclerosis is crucial. Nailfold capillaroscopy microscopy, vascular autoantibodies AT1R and ETAR, and several candidate-biomarkers have the potential to serve as noninvasive tools to identify systemic sclerosis patients at risk for developing pulmonary hypertension. Here, we explore the classifying potential of nailfold capillaroscopy microscopy characteristics and serum levels of selected candidate-biomarkers in a sample of systemic sclerosis patients with and without different forms of pulmonary hypertension.Methods: A total of 81 consecutive systemic sclerosis patients were included, 40 with systemic sclerosis pulmonary hypertension and 41 with no pulmonary hypertension. In each group, quantitative and qualitative nailfold capillaroscopy microscopy characteristics, vascular autoantibodies AT1R and ETAR, and serum levels of 24 soluble serum factors were determined. For evaluation of the nailfold capillaroscopy microscopy characteristics, linear regression analysis accounting for age, sex, and diffusing capacity of the lungs for carbon monoxide percentage predicted was used. Autoantibodies and soluble serum factor levels were compared using two-sample t test with equal variances.Results: No statistically significant differences were observed in quantitative or qualitative nailfold capillaroscopy microscopy characteristics, or vascular autoantibody ETAR and AT1R titer between systemic sclerosis-pulmonary hypertension and systemic sclerosis-no pulmonary hypertension. In contrast, several serum levels of soluble factors differed between groups: Endostatin, sVCAM, and VEGFD were increased, and CXCL4, sVEGFR2, and PDGF-AB/BB were decreased in systemic sclerosis-pulmonary hypertension. Random forest classification identified Endostatin and CXCL4 as the most predictive classifiers to distinguish systemic sclerosispulmonary hypertension from systemic sclerosis-no pulmonary hypertension.Conclusion: This study shows the potential for several soluble serum factors to distinguish systemic sclerosis-pulmonary hypertension from systemic sclerosis-no pulmonary hypertension. We found no classifying potential for qualitative or quantitative nailfold capillaroscopy microscopy characteristics, or vascular autoantibodies.

The field of applied linguistics is increasingly adopting open science practices. As open access publication gains traction, ethical issues emerge that need to be addressed by the field. This viewpoint paper addresses the concern that open science is not equally open for everyone. This paper describes how open access publication is increasingly being commercialized and explains how open access publication coincides with systemic inequality. We offer the following viewpoints for the field to consider:1.) We are morally obligated to make our research output accessible.2.) Hybrid, Gold, and Green open access publishing lead to systemic inequality in open access publishing, benefiting commercial publishers and those working in research-intensive universities and rich countries.3.)Diamond open access publication removes the systemic inequalities; hence, Diamond open access should be prioritized over Hybrid, Gold, and Green open access publication models.4.)We should move away from publish-and-read agreements and Green open access publishing, because they prevent system change.5.)Through our choices in our work as researchers, editors, reviewers, authors and teachers, we can contribute to the transition towards truly equitable open access publishing practices.6.)Senior researchers are in the position and have the moral obligation to be drivers of these changes.

Systemic sclerosis (SSc) is an autoimmune disease which is characterized by vasculopathy, tissue fibrosis and activation of the innate and adaptive immune system. Clinical features of the disease consists of skin thickening and internal organ involvement. Due to the heterogeneous nature of the disease it is difficult to predict disease progression and complications. Despite the discovery of novel autoantibodies associated with SSc, there is an unmet need for biomarkers for diagnosis, disease progression and response to treatment. To date, the use of single (surrogate) biomarkers for these purposes has been unsuccessful. Combining multiple biomarkers in to predictive panels or ultimately algorithms could be more precise. Given the limited therapeutic options and poor prognosis of many SSc patients, a better understanding of the immune-pathofysiological profiles might aid to an adjusted therapeutic approach. Therefore, we set out to explore immunological fingerprints in various clinically defined forms of SSc. We used multilayer profiling to identify unique immune profiles underlying distinct autoantibody signatures. These immune profiles could fill the unmet need for prognosis and response to therapy in SSc. Here, we present 3 pathophysiological fingerprints in SSc based on the expression of circulating antibodies, vascular markers and immunomodulatory mediators.

Growing research in sign language recognition, generation, and translation AI has been accompanied by calls for ethical development of such technologies. While these works are crucial to helping individual researchers do better, there is a notable lack of discussion of systemic biases or analysis of rhetoric that shape the research questions and methods in the field, especially as it remains dominated by hearing non-signing researchers. Therefore, we conduct a systematic review of 101 recent papers in sign language AI. Our analysis identifies significant biases in the current state of sign language AI research, including an overfocus on addressing perceived communication barriers, a lack of use of representative datasets, use of annotations lacking linguistic foundations, and development of methods that build on flawed models. We take the position that the field lacks meaningful input from Deaf stakeholders, and is instead driven by what decisions are the most convenient or perceived as important to hearing researchers. We end with a call to action: the field must make space for Deaf researchers to lead the conversation in sign language AI.

Local energy developments from a spatial and systemic approach are highlighted using examples from a Dutch case study. Developments in energy systems included interconnectedness of contextual factors and systems responses. The need to explore both the contextual factors and systemic aspects are illustrated. Using the case study of Energy Valley. Similarities and influences of local energy systems to the national and EU levels are also highlighted. In the aftermath of the systemic financial crises of 2007-9, several scholars argued that the problem of systemic financial crises is not well understood. At the same time, the introduction of digital technologies led to new threats and opportunities for the design of the monetary and financial system. For example, thousands of private cryptocurrencies have been implemented and hundreds of research papers on the (possible) introduction of public digital currencies have been published. It is often not explained why these new forms of digital money are needed and which (systemic) problems they (can) solve. In addition, the literature does not provide requirements nor guidelines to shape the development of the monetary and financial system in the digital age. This thesis applies design science to the monetary and financial system as a whole. The application of this novel methodology offers new possibilities to examine this complex system. The contribution of this thesis is threefold. First, different theories on money, banking and systemic financial crises have been researched through an extensive literature review and balance sheets. Second, those theories have been used to develop design requirements and guidelines. Finally, the consensus and pivotal dissensions about the systemic problem(s) of the current monetary and financial system, requirements and guidelines among experts have been identified through semistructured interviews. This research process results in widely supported requirements that demarcate the design space and widely supported guidelines that aim to give direction within the design space, that is, to the future development of the monetary and financial system.
