The paper presents a framework that through structured analysis of accident reports explores the differences between practice and academic literature as well amongst organizations regarding their views on human error. The framework is based on the hypothesis that the wording of accident reports reflects the safety thinking and models that have been applied during the investigation, and includes 10 aspects identified in the state-of-the-art literature. The framework was applied to 52 air accident reports published by the Dutch Safety Board (DSB) and 45 ones issued by the Australian Transport Safety Bureau (ATSB) from 1999 to 2014. Frequency analysis and statistical tests showed that the presence of the aspects in the accident reports varied from 32.6% to 81.7%, and revealed differences between the ATSB and the DSB approaches to human error. However, in overall safety thinking have not changed over time, thus, suggesting that academic propositions might have not yet affected practice dramatically.

In this study we demonstrate how accident and incident investigation reports can be useful to identify implicit safety views and accident investigation approaches. The framework was piloted through the analysis of sixteen (16) accident investigation reports published by a Nuclear Power Plant (NPP). The comments of independent researchers lead to framework refinements that increased the inter-rater reliability substantially. The initial results were validated through interviews with the staff of the NPP. Afterwards, the framework was applied to 52 air accident reports published by the Dutch Safety Board (DSB) from 1999 to 2013. Frequency calculations revealed the extent of new safety thinking embracement from the DSB, and Fisher’s Exact Test showed that none of the modern safety aspects has changed over time. The framework can be used to analyse accident investigation reports published by various organisations as means to identify implicit safety views and evolution of accident investigation practices over time. Further research will explore the reasons for potential gaps between theory and practice and contribute to minimizing such distance. Modern safety thinking and models focus more on systemic factors rather than simple cause-effect attributions of unfavourable events on the behaviour of individual system actors. This study concludes previous research during which we had traced practices of new safety thinking practices (NSTPs) in aviation investigation reports by using an analysis framework that includes nine relevant approaches and three safety model types mentioned in the literature. In this paper, we present the application of the framework to 277 aviation reports which were published between 1999 and 2016 and were randomly selected from the online repositories of five aviation authorities. The results suggested that all NSTPs were traceable across the sample, thus followed by investigators, but at different extents. We also observed a very low degree of using systemic accident models. Statistical tests revealed differences amongst the five investigation authorities in half of the analysis framework items and no significant variation of frequencies over time apart from the Safety-II aspect. Although the findings of this study cannot be generalised due to the non-representative sample used, it can be assumed that the so-called new safety thinking has been already attempted since decades and that recent efforts to communicate and foster the corresponding aspects through research and educational means have not yet yielded the expected impact. The framework used in this study can be applied to any industry sector by using larger samples as a means to investigate attitudes of investigators towards safety thinking practices and respective reasons regardless of any labelling of the former as “old” and “new”. Although NSTPs are in the direction of enabling fairer and more in-depth analyses, when considering the inevitable constraints of investigations, it is more important to understand the perceived strengths and weaknesses of each approach from the viewpoint of practitioners rather than demonstrating a judgmental approach in favour or not of any investigation practice.

The principal aim of this study is to explore the relations between work domains and the work-related learning of workers. The article is intended to provide insight into the learning experiences of Dutch police officers during the course of their daily work. Interviews regarding actual learning events and subsequent changes in knowledge, skills or attitudes were conducted with police officers from different parts of the country and in different stages of their careers. Interpretative analyses grounded in the notion of intentionality and developmental relatedness revealed how and in what kinds of work domains police officers appear to learn. HOMALS analysis showed work-related learning activities to vary with different kinds of work domains. The implications for training and development involve the role of colleagues in different hierarchical positions for learning and they also concern the utility of the conceptualisation of work-related learning presented here.

The range of studies that has been conducted on the role of gossip in organizations suggests that gossip in the workplace plays a variety of important roles in organisational processes. However, relatively few studies have explored its role in intercultural situations. This is surprising given how organisations are becoming increasingly diverse. This paper addresses this gap in the literature. It reports on an exploratory project that sought to determine how perceptions of organisational gossip vary between members of different cultural groups. Using a sensemaking, interpretative approach, we showed two gossip scenarios to 8 Chinese, 8 German and 8 Dutch first year students, and conducted semi structured interviews, asking them how they perceived the nature of the gossip, the gossiper and the object of gossip (i.e., the person being gossiped about). After analysing the data with ATLAS.ti, we observed certain patterns emerging. For example, while all students condemned a manager’s bad behaviour, the Chinese students seemed to expect it more than did their Dutch or German counterparts. Moreover, we found that the relationship and amount of trust that exists between gossiper, listener and object of gossip greatly influenced how the gossiper and object of gossip were perceived. After reflecting on our research methodology, this study sets the stage for the next phase of our research on the role of gossip in intercultural situations. Introduction: Worldwide, there is an increase in the extent and severity of mental illness. Exacerbation of somatic complaints in this group of people can result in recurring ambulance and emergency department care. The care of patients with a mental dysregulation (ie, experiencing a mental health problem and disproportionate feelings like fear, anger, sadness or confusion, possibly with associated behaviours) can be complex and challenging in the emergency care context, possibly evoking a wide variety of feelings, ranging from worry or pity to annoyance and frustration in emergency care staff members. This in return may lead to stigma towards patients with a mental dysregulation seeking emergency care. Interventions have been developed impacting attitude and behaviour and minimising stigma held by healthcare professionals. However, these interventions are not explicitly aimed at the emergency care context nor do these represent perspectives of healthcare professionals working within this context. Therefore, the aim of the proposed review is to gain insight into interventions targeting healthcare professionals, which minimise stigma including beliefs, attitudes and behaviour towards patients with a mental dysregulation within the emergency care context. Methods and analysis: The protocol for a systematic integrative review is presented, using the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols recommendations. A systematic search was performed on 13 July 2023. Study selection and data extraction will be performed by two independent reviewers. In each step, an expert with lived experience will comment on process and results. Software applications RefWorks-ProQuest, Rayyan and ATLAS.ti will be used to enhance the quality of the review and transparency of process and results. Ethics and dissemination: No ethical approval or safety considerations are required for this review. The proposed review will be submitted to a relevant international journal. Results will be presented at relevant medical scientific conferences.

Introduction: A trauma resuscitation is dynamic and complex process in which failures could lead to serious adverse events. In several trauma centers, evaluation of trauma resuscitation is part of a hospital's quality assessment program. While video analysis is commonly used, some hospitals use live observations, mainly due to ethical and medicolegal concerns. The aim of this study was to compare the validity and reliability of video analysis and live observations to evaluate trauma resuscitations. Methods: In this prospective observational study, validity was assessed by comparing the observed adherence to 28 advanced trauma life support (ATLS) guideline related tasks by video analysis to life observations. Interobserver reliability was assessed by calculating the intra class coefficient of observed ATLS related tasks by live observations and video analysis. Results: Eleven simulated and thirteen real-life resuscitations were assessed. Overall, the percentage of observed ATLS related tasks performed during simulated resuscitations was 10.4% (P < 0.001) higher when the same resuscitations were analysed using video compared to live observations. During real-life resuscitations, 8.7% (p < 0.001) more ATLS related tasks were observed using video review compared to live observations. In absolute terms, a mean of 2.9 (during simulated resuscitations) respectively 2.5 (during actual resuscitations) ATLS-related tasks per resuscitation were not identified using live observers, that were observed through video analysis. The interobserver variability for observed ATLS related tasks was significantly higher using video analysis compared to live observations for both simulated (video analysis: ICC 0.97; 95% CI 0.97-0.98 vs. live observation: ICC 0.69; 95% CI 0.57-0.78) and real-life witnessed resuscitations (video analyse 0.99; 95% CI 0.99-1.00 vs live observers 0.86; 95% CI 0.83-0.89). Conclusion: Video analysis of trauma resuscitations may be more valid and reliable compared to evaluation by live observers. These outcomes may guide the debate to justify video review instead of live observations.

Introduction: From the patient and staff perspective, care delivery for patients experiencing a mental health problem in ambulance and emergency department (ED) settings is challenging. There is no uniform and internationally accepted concept to reflect people with a mental health problem who require emergency care, be it for, or as a result of, a mental health or physical health problem. On initial presentation to the emergency service provider (ambulance or ED), the cause of their healthcare condition/s (mental health and/or physical health) is often initially unknown. Due to this (1) the prevalence and range of underlying causes (mental and/or physical) of the patients presenting condition is unknown; (2) misattribution of physical symptoms to a mental health problem can occur and (3) diagnosis and treatment of the initial somatic complaint and cause(s) of the mental/physical health problem may be hindered.This study will name and define a new concept: 'mental dysregulation' in the context of ambulance and ED settings. Methods and analysis: A Delphi study, informed by a rapid literature review, will be undertaken. For the literature review, a steering group (ie, persons with lived experience, ED and mental health clinicians, academics) will systematically search the literature to provide a working definition of the concept: mental dysregulation. Based on this review, statements will be generated regarding (1) the definition of the concept; (2) possible causes of mental dysregulation and (3) observable behaviours associated with mental dysregulation. These statements will be rated in three Delphi rounds to achieve consensus by an international expert panel (comprising persons with lived experience, clinicians and academics). Ethics and dissemination: This study has been approved by the Medical Ethical Committee of the University of Applied Sciences Utrecht (reference number: 258-000-2023_Geurt van der Glind). Results will be disseminated via peer-reviewed journal publication(s), scientific conference(s) and to key stakeholders.

In safety science and practice, there have been various safety models, each of them reflecting a particular approach to safety management and accident causality. The large variety of models suggested in literature and applied in practice serve the communication of diverse perspectives towards safety and the need to consider contextual factors, but it does not allow the establishment of a common language within and across organisations and industry sectors. Considering the potential benefits of talking a lingua franca when it comes to safety and inspired by the Standard Model used in particle physics and recent suggestions from relevant studies, we thought of exploring the possibility to introduce a Standard Safety Model (STASAM). As a first step, we focused on four representative safety and accident models widely used, discussed and debated: the Swiss Cheese Model, AcciMap, Functional Resonance Analysis Method (FRAM) and Systems-Theoretic Accident Model and Processes (STAMP). We reviewed literature which compares the particular models, and we listed the strengths and weaknesses of each as a means to set the grounds for the STASAM. The combinations of these models with a focus to host their advantages and avoiding their disadvantages led to a three-level STASAM. The concept STASAM was used in two random incident investigation reports to assess its applicability and visualisation against the original models. The results of the application along with the STASAM concept were reviewed by three safety professionals and three safety researchers. The comments received were in the positive direction and indicated the potential of establishing an inclusive and commonly accepted safety/accident model. The next research phase will be the additional review of the STASAM and its pilot application to a variety of safety events and systems as a means to test its reliability and strengthen its validity.

The aim of this study was to investigate if physiotherapists had knowledge and skills in applying Bobath-based therapy (BB), also referred to as Neurodevelopmental Treatment, in the care of stroke patients and if they generally used the therapy in daily practice. This is because of the important emphasis placed on documenting the extent of the therapy given to the client groups compared in outcome studies measuring the efficacy of therapeutic interventions. The study took place as an intervention check for a large outcome study measuring the effects of BB therapy. BB therapy had been implemented on six wards, whereas six other wards did not use this approach. The physiotherapists (n /38) knowledge and skills in making decisions about applying the BB principles in all wards was measured in two steps. In step 1, the physiotherapists received a questionnaire focusing on their physiotherapy strategy, and Bobath education. In step 2, they received a case vignette describing a stroke patient and questions concerning the content of the physiotherapy provided to this patient. An expert panel judged the therapists responses to the questions of both steps. Of the physiotherapists working in the BB wards, 14 (74%) therapists generally used BB principles, whereas four (21%) therapists did not (one was uncertain). Of the physiotherapists working in the non-BB wards (n /19), three (16%) did use BB therapy whereas 10 (52%) therapists did not use the therapy (six responses were missing). The study showed that within the BB wards, the physiotherapists had followed sufficient BB education, as judged by a panel of experts, and demonstrated the knowledge and skills in applying the BB therapy, whereas in the other wards they did not. BB wards could therefore participate in the experimental group of the study measuring the effects of the Bobath therapy, and the non-BB wards could serve as proper control wards.
