Background: Impaired upper extremity function due to muscle paresis or paralysis has a major impact on independent living and quality of life (QoL). Assistive technology (AT) for upper extremity function (i.e. dynamic arm supports and robotic arms) can increase a client’s independence. Previous studies revealed that clients often use AT not to their full potential, due to suboptimal provision of these devices in usual care. Objective: To optimize the process of providing AT for impaired upper extremity function and to evaluate its (cost-)effectiveness compared with care as usual. Methods: Development of a protocol to guide the AT provision process in an optimized way according to generic Dutch guidelines; a quasi-experimental study with non-randomized, consecutive inclusion of a control group (n = 48) receiving care as usual and of an intervention group (optimized provision process) (n = 48); and a cost-effectiveness and cost-utility analysis from societal perspective will be performed. The primary outcome is clients’ satisfaction with the AT and related services, measured with the Quebec User Evaluation of Satisfaction with AT (Dutch version; D-QUEST). Secondary outcomes comprise complaints of the upper extremity, restrictions in activities, QoL, medical consumption and societal cost. Measurements are taken at baseline and at 3, 6 and 9 months follow-up.

Learning teams in higher education executing a collaborative assignment are not always effective. To remedy this, there is a need to determine and understand the variables that influence team effectiveness. This study aimed at developing a conceptual framework, based on research in various contexts on team effectiveness and specifically team and task awareness. Core aspects of the framework were tested to establish its value for future experiments on influencing team effectiveness. Results confirmed the importance of shared mental models, and to some extent mutual performance monitoring for learning teams to become effective, but also of interpersonal trust as being conditional for building adequate shared mental models. Apart from the importance of team and task awareness for team effectiveness it showed that learning teams in higher education tend to be pragmatic by focusing primarily on task aspects of performance and not team aspects. Further steps have to be taken to validate this conceptual framework on team effectiveness.

In recent decades, magazine advertisers have used an increasing number of open ads. Open ads do not guide consumers towards a specific interpretation as traditional ads do, and they require more effort to decipher. An experiment was carried out to determine the effects of ad openness on the attitude towards the ad, brand beliefs, the attitude towards the brand, and the moderating role of ease of comprehension on the attitude towards the ad. We also assessed the effects of openness in relation to an individual's need for cognition. As expected, openness was found to have an overall negative effect on the attitude towards the ad, brand beliefs, and the attitude towards the brand. The negative effect of openness was related to the difficulty participants experience searching for an interpretation. Contrary to expectations, however, need for cognition did not moderate the effects of openness on the attitude towards the ad and the ability to interpret open ads.

Abstract: Teledentistry offers possibilities for improving efficiency and quality of care and supporting cost-effective healthcare systems. This umbrella review aims to synthesize existing systematic reviews on teledentistry and provide a summary of evidence of its clinical- and cost-effectiveness. A comprehensive search strategy involving various teledentistry-related terms, across seven databases, was conducted. Articles published until 24 April 2023 were considered. Two researchers independently reviewed titles, abstracts and full-text articles. The quality of the included reviews was critically appraised with the AMSTAR-2 checklist. Out of 749 studies identified, 10 were included in this umbrella review. Two reviews focusing on oral-health outcomes revealed that, despite positive findings, there is not yet enough evidence for the long-term clinical effectiveness of teledentistry. Ten reviews reported on economic evaluations or costs, indicating that teledentistry is cost-saving. However, these conclusions were based on assumptions due to insufficient evidence on cost-effectiveness. The main limitation of our umbrella review was the critically low quality of the included reviews according to AMSTAR-2 criteria, with many of these reviews basing their conclusions on low-quality studies. This highlights the need for high-quality experimental studies (e.g., RCTs, factorial designs, stepped-wedge designs, SMARTs and MRTs) to assess teledentistry’s clinical- and cost-effectiveness.

Background: The purpose of this study was to investigate the cost-effectiveness and budget impact of the Boston University Approach to Psychiatric Rehabilitation (BPR) compared to an active control condition (ACC) to increase the social participation (in competitive employment, unpaid work, education, and meaningful daily activities) of individuals with severe mental illnesses (SMIs). ACC can be described as treatment as usual but with an active component, namely the explicit assignment of providing support with rehabilitation goals in the area of social participation. Method: In a randomized clinical trial with 188 individuals with SMIs, BPR (n = 98) was compared to ACC (n=90). Costs were assessed with the Treatment Inventory of Costs in Patients with psychiatric disorders (TIC-P). Outcome measures for the cost-effectiveness analysis were incremental cost per Quality Adjusted Life Year (QALY) and incremental cost per proportional change in social participation. Budget Impact was investigated using four implementation scenarios and two costing variants. Results: Total costs per participant at 12-month follow-up were e 12,886 in BPR and e 12,012 in ACC, a non-significant difference. There were no differences with regard to social participation or QALYs. Therefore, BPR was not cost-effective compared to ACC. Types of expenditure with the highest costs were in order of magnitude: supported and sheltered housing, inpatient care, outpatient care, and organized activities. Estimated budget impact of wide BPR implementation ranged from cost savings to e190 million, depending on assumptions regarding uptake. There were no differences between the two costing variants meaning that from a health insurer perspective, there would be no additional costs if BPR was implemented on a wider scale in mental health care institutions. Conclusions: This was the first study to investigate BPR cost-effectiveness and budget impact. The results showed that BPR was not cost-effective compared to ACC. When interpreting the results, one must keep in mind that the cost-effectiveness of BPR was investigated in the area of social participation, while BPR was designed to offer support in all rehabilitation areas. Therefore, more studies are needed before definite conclusions can be drawn on the cost-effectiveness of the method as a whole.

Recently, the job market for Artificial Intelligence (AI) engineers has exploded. Since the role of AI engineer is relatively new, limited research has been done on the requirements as set by the industry. Moreover, the definition of an AI engineer is less established than for a data scientist or a software engineer. In this study we explore, based on job ads, the requirements from the job market for the position of AI engineer in The Netherlands. We retrieved job ad data between April 2018 and April 2021 from a large job ad database, Jobfeed from TextKernel. The job ads were selected with a process similar to the selection of primary studies in a literature review. We characterize the 367 resulting job ads based on meta-data such as publication date, industry/sector, educational background and job titles. To answer our research questions we have further coded 125 job ads manually. The job tasks of AI engineers are concentrated in five categories: business understanding, data engineering, modeling, software development and operations engineering. Companies ask for AI engineers with different profiles: 1) data science engineer with focus on modeling, 2) AI software engineer with focus on software development , 3) generalist AI engineer with focus on both models and software. Furthermore, we present the tools and technologies mentioned in the selected job ads, and the soft skills. Our research helps to understand the expectations companies have for professionals building AI-enabled systems. Understanding these expectations is crucial both for prospective AI engineers and educational institutions in charge of training those prospective engineers. Our research also helps to better define the profession of AI engineering. We do this by proposing an extended AI engineering life-cycle that includes a business understanding phase.

Background: Patient education, advice on returning to normal activities and (home-based) exercise therapy are established treatment options for patients with non-specific low back pain (LBP). However, the effectiveness of physiotherapy interventions on physical functioning and prevention of recurrent events largely depends on patient self-management, adherence to prescribed (home-based) exercises and recommended physical activity behaviour. Therefore we have developed e-Exercise LBP, a blended intervention in which a smartphone application is integrated within face-to-face care. E-Exercise LBP aims to improve patient self-management skills and adherence to exercise and physical activity recommendations and consequently improve the effectiveness of physiotherapy on patients’ physical functioning. The aim of this study is to investigate the short- (3 months) and long-term (12 and 24 months) effectiveness on physical functioning and cost-effectiveness of e-Exercise LBP in comparison to usual primary care physiotherapy in patients with LBP. Methods: This paper presents the protocol of a prospective, multicentre cluster randomized controlled trial. In total 208 patients with LBP pain were treated with either e-Exercise LBP or usual care physiotherapy. E-Exercise LBP is stratified based on the risk for developing persistent LBP. Physiotherapists are able to monitor and evaluate treatment progress between face-to-face sessions using patient input from the smartphone application in order to optimize physiotherapy care. The smartphone application contains video-supported self-management information, video-supported exercises and a goal-oriented physical activity module. The primary outcome is physical functioning at 12-months follow-up. Secondary outcomes include pain intensity, physical activity, adherence to prescribed (home-based) exercises and recommended physical activity behaviour, self-efficacy, patient activation and health-related quality of life. All measurements will be performed at baseline, 3, 12 and 24months after inclusion. An economic evaluation will be performed from the societal and the healthcare perspective and will assess cost-effectiveness of e-Exercise LBP compared to usual physiotherapy at 12 and 24months. Discussion: A multi-phase development and implementation process using the Center for eHealth Research Roadmap for the participatory development of eHealth was used for development and evaluation. The findings will provide evidence on the effectiveness of blended care for patients with LBP and help to enhance future implementation of blended physiotherapy.

Computer security incident response teams (CSIRTs) respond to a computer security incident when the need arises. Failure of these teams can have far-reaching effects for the economy and national security. CSIRTs often have to work on an ad hoc basis, in close cooperation with other teams, and in time constrained environments. It could be argued that under these working conditions CSIRTs would be likely to encounter problems. A needs assessment was done to see to which extent this argument holds true. We constructed an incident response needs model to assist in identifying areas that require improvement. We envisioned a model consisting of four assessment categories: Organization, Team, Individual and Instrumental. Central to this is the idea that both problems and needs can have an organizational, team, individual, or technical origin or a combination of these levels. To gather data we conducted a literature review. This resulted in a comprehensive list of challenges and needs that could hinder or improve, respectively, the performance of CSIRTs. Then, semi-structured in depth interviews were held with team coordinators and team members of five public and private sector Dutch CSIRTs to ground these findings in practice and to identify gaps between current and desired incident handling practices. Kijkend naar de ontwikkelingen in de medische en farmaceutische zorg, concludeer ik dat het belang van innovaties niet altijd in overeenstemming is met de snelheid waarmee die innovaties hun plek krijgen in het standaardhandelingsarsenaal van zorgverleners. Veranderingen in de zorg gaan vaak langzaam en doorbraken worden slecht herkend. De vraag is hoe dit komt. Er blijken vele factoren van invloed op het mogelijke succes van een innovatie. Van groot belang is het inzicht dat innoveren meer is dan iets bedenken en dan maar aannemen dat het wel zal worden opgepikt door de (potentiële) doelgroep. Het aan de man brengen (‘dissemineren’) van de innovatie is mede bepalend voor een succesvolle implementatie. In de farmaceutische zorg is voor deze overbruggingsfunctie een belangrijke rol weggelegd voor de farmakundige. Mijn lectoraat, dat is gekoppeld aan de opleiding Farmakunde, zal zich bezighouden met het onderzoek naar het proces om farmaceutische innovaties te dissemineren. In deze openbare les licht ik de context en consequenties van dit onderzoeksthema nader toe. Ik begin met een uitleg van de farmakundige en diens toegevoegde waarde in het werkveld (hoofdstuk 1), en vervolg met een korte beschrijving van recente veranderingen binnen de zorg (hoofdstuk 2). In het begeleiden van die veranderingen ligt een belangrijke meerwaarde van de farmakundige, en de missie van dit lectoraat. Daarna (hoofdstuk 3) beschouwen we het innoveren in de (farmaceutische) gezondheidszorg in meer detail. Hoofdstuk 4 geeft diverse handvatten voor het kiezen van de juiste interventies om de afstand tussen de innovator en de toekomstige gebruiker te overbruggen en zodoende de toegang voor de gebruiker tot de innovatie te verbeteren.
