In vrijwel alle onderzoeken wordt er veel kritiek geuit op het toezicht van De Nederlandse Bank en in mindere mate ook het ministerie van Financiën. Wordt het niet eens tijd voor een strafrechtelijk onderzoek naar falend toezicht ?

The paper presents a framework that through structured analysis of accident reports explores the differences between practice and academic literature as well amongst organizations regarding their views on human error. The framework is based on the hypothesis that the wording of accident reports reflects the safety thinking and models that have been applied during the investigation, and includes 10 aspects identified in the state-of-the-art literature. The framework was applied to 52 air accident reports published by the Dutch Safety Board (DSB) and 45 ones issued by the Australian Transport Safety Bureau (ATSB) from 1999 to 2014. Frequency analysis and statistical tests showed that the presence of the aspects in the accident reports varied from 32.6% to 81.7%, and revealed differences between the ATSB and the DSB approaches to human error. However, in overall safety thinking have not changed over time, thus, suggesting that academic propositions might have not yet affected practice dramatically.

Safety is monitored by various proactive and reactive methods, including the investigation of adverse accidents and incidents, which are collectively known as safety investigations. In this study we demonstrate how accident and incident investigation reports can be useful to identify implicit safety views and accident investigation approaches. An analysis framework was developed based on contemporary safety literature. The framework incorporates aspects such as hindsight bias, judgemental approach, proximal or distal focus, and the application of systemic versus sequential accident causation models. The framework was piloted through the analysis of sixteen (16) accident investigation reports published by a Nuclear Power Plant (NPP). The comments of independent researchers lead to framework refinements that increased the inter-rater reliability substantially. The initial results were validated through interviews with the staff of the NPP. Afterwards, the framework was applied to 52 air accident reports published by the Dutch Safety Board (DSB) from 1999 to 2013. Frequency calculations revealed the extent of new safety thinking embracement from the DSB, and Fisher’s Exact Test showed that none of the modern safety aspects has changed over time. The framework can be used to analyse accident investigation reports published by various organisations as means to identify implicit safety views and evolution of accident investigation practices over time. Further research will explore the reasons for potential gaps between theory and practice and contribute to minimizing such distance. Twee Nederlandse Toezichthouders, de Nederlandse Bank en de Autoriteit Financiële Markten hebben een onderzoek ingesteld naar de rol van G.Zalm en E.Nijpels tijdens hun functioneren als financiëel directeur respectievelijk commissaris bij DSB. Dat leverde tegenstrijdige adviezen op voor G.Zalm die later rechtgestreken werden door de commissie Scheltema.Toch blijven er grote vraagtekens en zeker met het heimelijk vertrek van E. Nijpels.

Breedtesport en topsport ontwikkelen zich in een verschillende richting, maar blijven elkaar beinvloeden. Deze beinvloeding kan worden onderscheiden naar effecten die samenhangen met de veronderstelde aanvoerfunctie van de breedtesport en effecten die voortkomen uit de veronderstelde aanjaagfunctie van de topsport. Beide functies maken onderdeel uit van de zogenoemde 'double pyramid theory'. Volgens deze theorie brengen duizenden sportbeoefenaars een paar Olympische kampioenen voort (aanvoerfunctie), terwijl deze kampioenen door hun rolmodel weer duizenden mensen stimuleren om één of andere vorm van sport te gaan beoefenen (aanjaagfunctie). Onderzoek wijst echter uit dat er geen sprake is van een gelijke mate van beïnvloeding tussen topsport en breedtesport. De topsport kan niet zonder breedtesport, maar andersom is dit in veel mindere mate het geval. Breedtesporters ontlenen vooral als publiek veel plezier aan de topsport, maar een groot deel van de sportbeoefenaars, vooral buiten verenigingsverband, zal in hun sportbeoefening nauwelijks een relatie met de topsport ervaren.

Dit boek geeft een uitgebreide beschrijving en interpretatie van de kredietcrisis zoals die in het najaar van 2008 plaats vond. Het bevat enerzijds veel informatie over de kredietcrisis en anderzijds een veelzijdigheid aan onderwerpen zoals: waarderingsgrondslagen, financieringsstructuren, informatie, marketing (vooral promotie, sponsoring, merkenstrategie), integriteit, moral hazard, wilsgebreken, aansprakelijkheid, politiek, risico's, risicomanagement en beloningsstructuren. Het onderwerp beslaat de basis van onze economie, namelijk het vertrouwen in onze samenleving en daarmee transparantie en integriteit. De veelzijdigheid van het onderwerp, de theoretische onderbouwingen, de vele praktijkcomponenten en de vele relevante afbeeldingen, maken van dit manifest een boek over de economie in praktijk. Doelgroepen voor dit zijn: de gedupeerden, de politici en beslissers in financiële instellingen, studenten en middelbare scholieren en alle overige geïnteresseerden. Om het boek voor een breed publiek toegankelijk te houden, is de materie op een eenvoudige wijze beschreven waarbij gebruik wordt gemaakt van veel afbeeldingen en eenvoudig taalgebruik.

De afgelopen jaren is in de media in toenemende mate bericht en gerapporteerd door journalistiek en toezichthouder over de wijze waarop accountants hun rol als “hoeder van het maatschappelijk verkeer”vervullen. Corruptieschandalen (Ballast Nedam), geknoei met cijfers en waarderingen (Ahold, Vestia, Steinhoff) en bedrijven die onder het toeziend oog van accountants failliet gingen (DSB en IMtech) hebben ertoe bijgedragen dat het accountantsberoep in zwaar weer is terecht gekomen en dat de competentie van degenen die het beroep uitoefenen in twijfel wordt getrokken. De vraag in vervolg op het eerder in TEO geplaatste artikel (Soft Skills deel 1) is in hoeverre bedrijfseconomische en accountant opleidingen kunnen bijdragen aan een noodzakelijke verandering.
