Cannabis is commonly characterized as the most normalized illicit drug. However, it remains a prohibited substance in most parts of the world, including Europe, and users can still be subject to stigmatization. The objective of this study is to assess to what extent and how cannabis users in different countries with different cannabis policies perceive, experience and respond to stigmatization. We conducted a survey in the Dutch coffeeshops among 1225 last year cannabis users from seven European countries, with national cannabis policies ranging from relatively liberal to punitive. Three dimensions of cannabis-related stigma were investigated (discrimination, perceived devaluation and alienation) and a sum score was used to reflect the general level of stigmatization. Perceived devaluation was the highest-scoring dimension of stigma and discrimination the lowest-scoring. The general level of stigmatization was lowest in the Netherlands and highest in Greece. This indicates that punitive cannabis policy is associated with stigma and liberal cannabis policy is associated with de-stigmatization. Besides country, daily cannabis use was also found to be a significant predictor of stigma, but gender, age, household type and employment status were not.

Introduction: Worldwide, there is an increase in the extent and severity of mental illness. Exacerbation of somatic complaints in this group of people can result in recurring ambulance and emergency department care. The care of patients with a mental dysregulation (ie, experiencing a mental health problem and disproportionate feelings like fear, anger, sadness or confusion, possibly with associated behaviours) can be complex and challenging in the emergency care context, possibly evoking a wide variety of feelings, ranging from worry or pity to annoyance and frustration in emergency care staff members. This in return may lead to stigma towards patients with a mental dysregulation seeking emergency care. Interventions have been developed impacting attitude and behaviour and minimising stigma held by healthcare professionals. However, these interventions are not explicitly aimed at the emergency care context nor do these represent perspectives of healthcare professionals working within this context. Therefore, the aim of the proposed review is to gain insight into interventions targeting healthcare professionals, which minimise stigma including beliefs, attitudes and behaviour towards patients with a mental dysregulation within the emergency care context. Methods and analysis: The protocol for a systematic integrative review is presented, using the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols recommendations. A systematic search was performed on 13 July 2023. Study selection and data extraction will be performed by two independent reviewers. In each step, an expert with lived experience will comment on process and results. Software applications RefWorks-ProQuest, Rayyan and ATLAS.ti will be used to enhance the quality of the review and transparency of process and results. Ethics and dissemination: No ethical approval or safety considerations are required for this review. The proposed review will be submitted to a relevant international journal. Results will be presented at relevant medical scientific conferences.

Purpose: Adolescents are the least likely to seek help for their mental health problems. School may be an important route to improve early recognition of adolescents with mental health problems in need for support, but little is known about the barriers to school support.Materials and methods: Data were collected in a longitudinal cohort study of Dutch adolescents (age 12–16) in secondary school (n = 956). We assessed the relation between level of psychosocial problems at the beginning of the school year (T1) and the support used in school at the end of that school year (T2), whether the willingness to talk to others (measured at T1) mediates this relation, and whether stigma towards help-seeking (T1) moderates this mediation.Results: Adolescents with more psychosocial problems were more likely to use support in school and were less willing to talk to others about their problems, but the willingness to talk to others was not a mediator. Stigma moderated the relationship between psychosocial problems and willingness to talk to others.Conclusions: Most adolescents with psychosocial problems get support in Dutch secondary school regardless of their willingness to talk to others about their problems. However, perceiving stigma towards help-seeking makes it less likely for someone to talk about their problems.

Bariatric surgery is an effective procedure for the treatment of obesity. Despite this, only 0.1% to 2% of eligible individuals undergo surgery worldwide. The stigma surrounding surgery might be a reason for this.This study aimed to explore the perceptions, experiences, and consequences of bariatric surgery stigma from the perspective of the public, health care professionals, and patients before and after bariatric surgery. Furthermore, although the concept of stigma is universal, every society has specific cultural norms and values that define acceptable attributes and behaviors for its members. Therefore, this study also aimed to explore the extent to which cultural factors influence bariatric surgery stigma by comparing the Netherlands, France, and the United Kingdom.

Despite high prevalence of mental health problems among university students, there’s a gap between the need for help and the actual treatment received. This studyinvestigated disclosure on distress and hazardous alcohol use and help-seeking behavior in a sample of 1,791 students of a Dutch university of applied sciences.Students’ perceived public and personal stigma, and attitudes towards disclosure and help-seeking were assessed as possible predictors of disclosure and help-seekingbehavior. Results of the analysis of variance and logistic regression analysis indicated that perceived public and personal stigma did not predict disclosure and helpseeking behavior, but that attitudes towards disclosure and help-seeking did. Students with both distress and hazardous alcohol use have the least tendency to disclosetheir problems to family, friends or classmates, but at the same time they do tend to seek help. Disclosure and seeking help for mental health challenges are healthpromoting competencies that seem to need more attention in university students. Although further research needs to validate these findings, it is recommended topromote disclosure and help-seeking among students by investing in mental health literacy programs, to educate students about mental health issues, raise awarenesson available mental health services and their potential benefits.

Het gebruik van ervaringskennis onder behandelaren in de geestelijke gezondheidszorg (ggz) is een opkomend fenomeen. Een significant percentage van alle ggz-professionals heeft zélf te maken gehad met psychische ontregeling. Studies suggereren uiteenlopende percentages in de groep ggz-professionals, variërend van 45 procent tot zeventig procent, wat hoger is dan gemiddeld genomen in de beroepsbevolking. Velen voelen zich juist ook om deze reden aangetrokken tot het hulpverlenerschap, hoewel slechts een minderheid expliciet gebruikmaakt van deze kennis. In opleidingen en nascholingen werd oorspronkelijk ruim aandacht besteed aan de risico’s van zelfonthulling. Daarnaast blijkt uit onderzoek dat het klimaat tijdens leertherapie en intervisies niet altijd veilig genoeg is om persoonlijke ervaringen met ontwrichting bespreekbaar te maken. Als dit wel het geval is, dan ligt de nadruk op het inzichtelijk krijgen van de eigen problematiek zodanig dat deze niet interfereert met toekomstig cliëntcontact. Hiermee wordt vaak uitgegaan van een mogelijk negatieve invloed van ervaringskennis. Eenmaal werkzaam in de ggz is het eveneens niet vanzelfsprekend om er openlijk over te spreken met collega’s en is er regelmatig sprake van (zelf-)stigma. Er bestaan namelijk nog veel misvattingen over het gebruik van ervaringskennis en -deskundigheid, voornamelijk voortkomend uit de psychoanalyse, zoals het idee dat het schadelijk zou zijn voor cliënten. Ggz-professionals hebben de neiging zich te verschuilen achter hun professionele identiteit, met vaak weinig ruimte voor persoonlijke aspecten. Daarbij komt dat er een trend is om in toenemende mate de interventie boven de relatie te prevaleren. Terwijl het belang van ervaringskennis in behandelingen internationaal en ook in Nederland steeds meer wordt erkend, raken psychotherapeuten op achterstand. Beroepsorganisaties als het Royal Australian and New Zealand College of Psychiatrists en de British Psychological Society benadrukken juist dat persoonlijke ervaringen een essentiële rol kunnen spelen bij het verminderen van stigma. In Nederland heeft de Nederlandse Vereniging voor Psychiatrie onlangs kenbaar gemaakt dit als een positieve ontwikkeling te zien. In Nederland is tevens een kwaliteitsstandaard en zorgstandaard voor het gebruik van ervaringsdeskundigheid en worden ervaringsdeskundigen steeds meer gewaardeerd vanwege hun unieke bijdragen. Terwijl dus steeds meer ex-cliënten worden opgeleid om hun ervaringen met psychische problemen en herstel in te zetten om anderen te helpen, wordt het psychologen nog steeds afgeleerd om open te zijn over hun persoonlijke verleden. Zodoende startte in 2019 een promotieonderzoek naar het gebruik van ervaringsdeskundigheid onder ggz-professionals. Gedurende ruim vier jaar werd kwalitatief en participatief onderzoek uitgevoerd bij vier zorgorganisaties. Naast een literatuurstudie werd er ook praktijkgericht samengewerkt met cliënten, ervaringsdeskundigen, professionals, bestuurders en managers. Een belangrijke vraag hierin was: ‘Wat levert professionele ervaringskennis binnen een therapeutische setting op en hoe kan dit door ggz-professionals worden ingezet?’

Online support communities are gaining attention among child-attracted persons (CAPs). Though research has largely focused on the negative consequences these environments create for potential offending, they may also provide a beneficial alternative to more formal treatment settings. To assess the utility for clinical and therapeutic purposes, this analysis focused on subcultural dynamics to examine self-reported wellbeing outcomes of participation in a Dutch forum for CAPs. A total of 15 semi-structured interviews were conducted with moderators, members and mental health professionals involved in the community. Thematic analyses demonstrated that by means of informal social control, bonds of trust and social relational education, the network aims to regulate the behavior and enhance the wellbeing of its marginalized participants. Key outcomes include a decreased sense of loneliness and better coping with stigma, to the point that participants experience less suicidal thoughts. Association with prosocial peers also helps to set moral boundaries regarding behavior towards children, although we cannot fully rule out potential adverse influences. Online support networks offer a stepping stone to professional care that fits individual needs of CAPs, while also providing an informal environment that overcomes limitations of physical therapy and that extents principles of existing prevention and desistance approaches. Achtergrond: Patiënten met verslavingsproblematiek ervaren vaak zelfstigma. Steun van de omgeving lijkt zelfstigma te verminderen, maar nog weinig is bekend over de relatie met ervaren en gewenste sociale steun.Doel: Zicht krijgen op de samenhang tussen ervaren sociale steun, gewenste sociale steun en zelfstigma bij patiënten met verslavingsproblematiek en te onderzoeken of de ernst van de verslaving hierin een rol speelt. Methode: 83 ambulante patiënten met verslavingsproblematiek vulden de Sociale Steun Lijst in en de Internalized Stigma of Mental Illness scale.Resultaten: Pearson correlaties toonden verbanden tussen gewenstheid van sociale steun, ervaren sociale steun en zelfstigma, maar een multipele regressie liet alleen een verband zien tussen gewenstheid van sociale steun en zelfstigma. Het belang van gewenstheid van sociale steun werd ook gevonden bij patiënten met een lage verslavingsernst, maar bij patiënten met een ernstige verslaving werd juist een verband gevonden met de ervaren sociale steun.Conclusie: Gewenste sociale steun lijkt meer van belang te zijn voor zelfstigma dan de ervaren sociale steun. Bij mensen met een ernstige verslaving lijkt ervaren sociale steun echter belangrijker te zijn.
