Background. Violent criminal offenders with personality disorders (PD’s) can cause immense harm, but are often deemed untreatable. This study aimed to conduct a randomized clinical trial to test the effectiveness of long-term psychotherapy for rehabilitating offenders with PDs. Methods. We compared schema therapy (ST), an evidence-based psychotherapy for PDs, to treatment-as-usual (TAU) at eight high-security forensic hospitals in the Netherlands. Patients in both conditions received multiple treatment modalities and differed only in the individual, study-specific therapy they received. One-hundred-three male offenders with antisocial, narcissistic, borderline, or paranoid PDs, or Cluster B PD-not-otherwise specified, were assigned to 3 years of ST or TAU and assessed every 6 months. Primary outcomes were rehabilitation, involving gradual reintegration into the community, and PD symptoms. Results. Patients in both conditions showed moderate to large improvements in outcomes. ST was superior to TAU on both primary outcomes – rehabilitation (i.e. attaining supervised and unsupervised leave) and PD symptoms – and six of nine secondary outcomes, with small to moderate advantages over TAU. ST patients moved more rapidly through rehabilitation (supervised leave, treatment*time: F(5308) = 9.40, p < 0.001; unsupervised leave, treatment*- time: F(5472) = 3.45, p = 0.004), and showed faster improvements on PD scales (treatment*- time: t(1387) = −2.85, p = 0.005). Conclusions. These findings contradict pessimistic views on the treatability of violent offenders with PDs, and support the effectiveness of long-term psychotherapy for rehabilitating these patients, facilitating their re-entry into the community

Patienten in der forensischen Psychiatrie haben eine schwere Straftat begangen, für die sie aufgrund ihrer psychiatrischen Probleme nicht oder nur teilweise strafrechtlich verantwortlich sind. Bei ihnen ist also nachweislich sozusagen „eine Schraube locker“. Auch die Menschen, die dort arbeiten, haben nach der öffentlichen Meinung einen Makel: „Man muss verrückt sein, um dort arbeiten zu wollen“. Das Bild ist in der Regel nicht positiv, vor allem, wenn Vorfälle in den Medien auftauchen, bei denen das behandelnde Personal oft als inkompetent abgetan wird. Oder gibt es vor allem Mängel in den Abläufen und der Organisation der forensischen Versorgung? „Das System steht still“, titelte die niederländische Rundfunkanstalt NOS im Dezember 2023 eine ihrer Nachrichtensendungen. Es besteht Personalknappheit, eine hohe Arbeitsbelastung und lange Wartelisten. Außerdem ist es schwierig, die Weiterversorgung so zu organisieren, dass die Kontinuität der Betreuung gewährleistet ist. Wie groß sind diese Probleme wirklich, und sind sie reparabel? Bei der näheren Betrachtung aus drei Perspektiven – Patienten, Fachkräfte und Prozesse – kommt die Autorin zu dem Schluss, dass sich parallele Themen erkennen lassen, und eine integrierte Perspektive erforderlich ist, um die Qualität der forensischen Versorgung weiter zu verbessern.

PURPOSE: To test if a collaborative care program (CCP) with nurses in a coordinating position is beneficial for patients with severe personality disorders. DESIGN AND METHODS: A pilot study with a comparative multiple case study design using mixed methods investigating active ingredients and preliminary results. FINDINGS: Most patients, their informal caregivers, and nurses value (parts of) the CCP positively; preliminary results show a significant decrease in severity of borderline symptoms. PRACTICE IMPLICATIONS: With the CCP,we may expand the supply of available treatments for patients with (severe) personality disorders, but a larger randomized controlled trial is warranted to confirmour preliminary results.

Van Jeugdzorg naar oplossingen beschrijft de mogelijkheden om door samenwerking met jeugdzorg en onderwijs het product te verbeteren en tevens de waarde van de sportvereniging te verhogen voor alle belanghebbenden. Sport is een plezierige belevenis door Physieke Prestaties met Partners. Daarnaast bevat sport een groot scala van additionele producten waarmee de persoonlijke ontwikkeling van kinderen kunnen worden verbeterd, alsmede de socialisatie en nog veel maatschappelijke bijproducten. Door de verbetering van deze additionele waardes ontstaat vanzelf een beter hoofdproduct. Onderwijs en Jeugdzorg kunnen hier een belangrijke bijdrage in leveren. Anderzijds kan de sportvereniging een belangrijke tegenprestatie leveren ten gunste van de ontwikkeling en het geluk van kinderen. Hierdoor krijgt de subsidie een draagvlak waarmee verenigingen beoordeeld kunnen worden. Synergie is hierbij het sleutelwoord, door samenwerking ontstaan nieuwe processen of leiden ze tot kostenreductie. De productiviteit en dus de meerwaarde van verenigingen kan toenemen door deze verhoogde effectiviteit en efficiency. Organisatorisch zal er ruimte moeten worden gemaakt voor de pedagogisch- en zorg-coördinator. Met een beter informatiesysteem krijgt jeugd een meer centrale positie die sturend kan worden voor de ontwikkeling. Per regio of zorggebied kan een intensieve samenwerking worden gestimuleerd van zelfsturende teams met doelstellingen op projectbasis. Vanuit het onderwijs en de jeugdzorg kan de participatie bij jeugdverenigingen worden gestimuleerd. Verenigingen krijgen nu veel, volle en vaste leden/ vrijwilligers die vaker komen waarmee de positionering duidelijker wordt.

Background The Self-Expression Emotion Regulation in Art Therapy Scale (SERATS) was developed as art therapy lacked outcome measures that could be used to monitor the specific effects of art therapy. Although the SERATS showed good psychometric properties in earlier studies, it lacked convergent validity and thus construct validity. Method To test the convergent validity of the SERATS correlation was examined with the EES (Emotional Expressivity Scale), Emotion Regulation Strategies for Artistic Creative Activities Scale (ERS-ACA) and Healthy-Unhealthy Music Scale (HUMS). Patients diagnosed with a Personality Disorder, and thus having self-regulation and emotion regulation problems (n = 179) and a healthy student population (n = 53) completed the questionnaires (N = 232). Results The SERATS showed a high reliability and convergent validity in relation to the ERS-ACA approach strategies and self-development strategies in both patients and students and the HUMS healthy scale, in patients. Hence, what the SERATS measures is highly associated with emotion regulation strategies like acceptance, reappraisal, discharge and problem solving and with improving a sense of self including self-identity, increased self-esteem and improved agency as well as the healthy side of art making. Respondents rated the SERATS as relatively easy to complete compared to the other questionnaires. Conclusion The SERATS is a valid, useful and user-friendly tool for monitoring the effect of art therapy that is indicative of making art in a healthy way that serves positive emotion regulation and self-development.

AIM: To synthesize the evidence about the characteristics (frequency, intensity, time, type) and effects of physical rehabilitation interventions on functional recovery and performance in daily functioning in children and young people with acquired brain injury (ABI), including traumatic brain injuries (TBI) and non-TBI, during the subacute rehabilitation phase.METHOD: Using scoping review methodology, a systematic literature search was performed using four databases. Articles were screened by title and abstract and data from eligible studies were extracted for synthesis.RESULTS: Nine of 3009 studies were included. The results demonstrated a variety of intervention characteristics: frequency varied between 1 and 7 days per week; time of intervention varied between 25 minutes and 6 hours a day; intervention types were specified in seven studies; and none of the included studies reported details of intensity of intervention. All studies reported positive results on the International Classification of Functioning, Disability and Health: Children and Youth (ICF-CY) levels of body function and activities after the intervention period, with study designs of included studies being cohort studies without concurrent controls (n=7) or case reports (n=2).INTERPRETATION: Inconsistency in results hampers generalizability to guide clinical practice. Physical interventions during subacute rehabilitation have potential to improve functional recovery with intervention characteristics as an important factor influencing its effectiveness. Future well-designed studies are indicated to gain knowledge and optimize rehabilitation practice in paediatric ABI and high-quality research including outcomes across all ICF-CY domains is needed.

Introduction: Gastroenteropancreatic neuroendocrine neoplasms (GEPNENs) are often diagnosed in an advanced stage. As the optimal sequence of therapy remains largely unclear, all treatment-related outcomes, including health-related quality of life (HRQoL) prospects, should be assessed according to patients' preferences. Methods: A targeted search was performed in PubMed and EMBASE to identify studies on treatment effect and HRQoL, measured using the EORTC QLQ-C30 tool, in patients with advanced, well-differentiated GEPNENs. Study quality was assessed, and meta-analyses were performed for global health status/QOL and tumour response. Results: The search yielded 1,322 records, and 20 studies were included, examining somatostatin analogues (SSA), peptide receptor radionuclide therapies (PRRT), chemotherapy, SSA-based combination therapies, and targeted therapies. Global HRQoL was stable, and rates for disease stabilisation were moderate to high across all treatments. Meta-analyses for global health status/QOL after SSA treatment were not significant (mean difference: –0.3 [95% CI: −1.3 to 0.7]). The highest pooled overall tumour response rate was 33% (95% CI: 24–45%) for PRRT. The highest pooled clinical benefit rate was 94% (95% CI: 65–99%) for chemotherapy. Conclusion: All treatments appeared beneficial for disease stabilisation while maintaining stable global health status/QOL. High-quality HRQoL reporting was lacking. HRQoL should be a central outcome next to well-established outcomes.

To help prevent its use, professionals need insight into what nurses believe about coercion and which staff determinants may influence its application. There is need for an integrated review on both attitude and influence of nurses on the use of coercion.4.2 AimTo summarize literature concerning attitude of nurses towards coercive measures and the influence of staff characteristics on the use of coercive measures.4.3 MethodSystematic review.4.4 ResultsThe attitude of nurses changed during the last two decades from a therapeutic to a safety paradigm. Nurses currently view coercive measures as undesirable, but necessary to deal with aggression. Nurses express the need for less intrusive interventions, although familiarity probably influences its perceived intrusiveness. Literature on the relation between staff characteristics and coercive measures is inconclusive.4.5 DiscussionNurses perceive coercive measures as unwanted but still necessary to maintain safety on psychiatric wards. Focussing on the determinants of perception of safety might be a promising direction for future research.4.6 Implications for practiceMental health care could improve the focus on the constructs of perceived safety and familiarity with alternative interventions to protect patients from unnecessary use of coercive interventions.
